ObjectivesParkinson’s disease is a common neurodegenerative disease,studies have confirmed that PD lesions are’t only localized at extrapyramidal area, but also at the cortex. Voxel based morphometry method is a morphometric methods based on the voxel as a unit, is describe for quantitative detection of density and volume of brain tissue.ReHo is used to assess the situation of spontaneous neuronal activity by measure the similarity of BOLD signal fluctuations in the time series of a given voxel to those of its nearest neighbors in the same time sequence.ALFF as a measuring the amplitude of spontaneous activity of neurons method,to reflect the level of spontaneous activity in rest state.ObjectiveThe aim of this study is to explore the change of Structural and Rest-state functional MRI features in Parkinson’s disease by VBM method、ReHo and ALFF method, and explore the relationship between image change and the serious situation in movement disorder.MethodsOutpatients with Parkinson’s disease from our hospital between September2013and March2014and volunteers were recruited. General Information, UPDRS Ⅲ score, Hoehn-Yahr stage and MRI were recorded. Image processing statistical analysis were performed using related software.A two-sample t-test was performed.Results were displayed with a threshold of P <0.01,corrected by FDR correction and cluster size>30voxels.Results1Compared with the NC,PD exhibit reduced gray matter volume in the Postcentral Gyrus, the right SupraMarginal, Superior Temporal Gyrus, Precentral Gyrus, brodmann area41, Transverse Temporal Gyrus, brodmann area3, Inferior Parietal Lobule.(two-sample T-test, P <0.01, FDR corrected)2Compared with the NC,PD exhibit increased AlFF value in limbic lobe,the right parahippocampal gyrus, the right midlle temporal_pole; reduced ALFF values in the left thalamus (two sample T-test, P <0.01, FDR corrected).3Compared with the NC,PD exhibit increased ReHo value in the superior medial frontal gyrus, the left middle frontal gyrus, brodmann area9, brodmann area6; reduced ALFF value in the left rolandic,the left superior temporal gyrus, insula lobe(two sample T-test, P <0.01, FDR corrected)4Correlation analysis: the negatively correlated between the cerebral gray matter volume changes and UPDRS Ⅲ score in the right middle frontal gyrus and right brodmann area6, the right precentral gyrus,the right superior frontal gyrus,the medial frontal gyrus.The negatively correlated between the cerebral gray matter volume changes and Hoehn-Yahr state in the middle frontal gyrus, the right superior frontal gyrus,the right brodmann area6, the right precentral gyrus(P <0.01, FDR corrected). ALFF value positively correlated with the UPDRS Ⅲ score in the right posterior Cerebellum lobe, cerebellar tonsil (P <0.02, FDR corrected). ALFF value and HY negative return on the superior frontal gyrus(P <0.01, FDR corrected). The negatively correlated between the ReHo value and UPDRS Ⅲ score in the inferior triangular frontal area,the right superior temporal gyrus and the right inferior orbitofrontal area, BA47, the right superior temporal pole, insular cortex, limbic lobe, corpus callosum, middle temporal gyrus, caudate nuclear (P <0.03, FDR correction). The positively correlated between the ReHo value and H-Y scale in medial frontal gyrus,limbic lobe, the anterior cingulate, medial orbitofrontal area, the middle superior frontal gyrus, BA32, the left superior frontal gyrus, BA10(P <0.03, FDR corrected).ConclusionThis study found that extensive changes in the grey matter volume, mainly located in related area of movement, which had obvious relevance with the progression of the disease.By resting state functional MRI,we found abnormal brain function in Parkinson’s disease, ReHo, ALFF showed abnormal changes in the connection of neuronal networks. The compensatory response may lead to the increased brain function.
