Objective:1.Reho and ALFF methods were used to investigate the changes and significance of spontaneous neuronal function in patients with Parkinson’s disease with cognitive dysfunction.2.Voxel-wise FC method was used to investigate the changes of brain functional connectivity patterns and their pathological significance in patients with Parkinson’s disease with cognitive dysfunction.3.To investigate the functional connectivity among networks and its significance in patients with PD with cognitive dysfunction by using ICA to isolate multiple brain networks and evaluate the connectivity among networks by FNC.Materials and methods:Thirty-four patients with primary PD were included and collected,and the diagnostic criteria were based on the diagnostic guidelines of Parkinson’s disease of the Chinese Medical Association.During the same period,34 healthy people with age,gender and education matching PD patients were included and collected.Among the PD group,20 patients with Mini-mental State Examination,simple mental State Examination scale(MMSE)< 27 points were regarded as cognitive disorder(CI),forming PD-CI group:1.The data were analyzed by local consistency(REHO)and low frequency amplitude(ALFF)analysis methods,and the changes of brain functional activities in PD patients and PD patients with cognitive impairment were observed.Two-sample t-test was used for comparison between groups.PD-CI group and HC group were compared pairwise.2.The functional link method was used to analyze the results,and the functional link changes between the whole brain and the right orbital superior frontal gyrus and the right orbital middle frontal gyrus were obtained in each group.3.ICA method was used to decompose the whole brain independent components,and the functional connectivity(FNC)among independent components was calculated by taking independent components as unit.Results:1.Compared with HC group,the Reho values of right orbitofrontal gyrus,right inferior frontal gyrus of orbit,left superior temporal gyrus and left thalamus in PD group were increased,while the Reho values of left peri-Torque fissure cortex,right suproccipital gyrus,left middle frontal gyrus,right middle occipital gyrus,left middle occipital gyrus and left precuneus were decreased.In the PD-CI group,the Reho values of the left superior temporal gyrus and the left thalamus were increased,while the Reho values of the left peripheral cortex of the torque fissure and the right suprapoccipital gyrus were decreased.Compared with HC group,ALFF values of right orbitofrontal gyrus,right orbitofrontal gyrus and left orbitofrontal gyrus in PD group were increased,while ALFF values of right inferior temporal gyrus,right middle temporal gyrus,left middle temporal gyrus,right lactiform putamen,right glossal gyrus,left thalamus,left triangle inferior frontal gyrus and right posterior central gyrus were decreased.In PD-CI group,ALFF values of left orbital inferior frontal gyrus,left orbital middle frontal gyrus and right orbital inferior frontal gyrus were increased,while ALFF values of right lenticular putamen,left lenticular putamen and right precuneus were decreased.2.Compared with HC group,the brain regions with enhanced connections between PD group and right orbital middle frontal gyrus and right orbital middle frontal gyrus were left anterior cingulate cingulate gyrus and parapingulate gyrus.Compared with HC group,PD-CI group had enhanced connections to the left anterior cingulate and parapingulate gyrus,brain regions with enhanced connections to the right orbital middle frontal gyrus and the right orbital middle frontal gyrus.3.independent component analysis,the extraction filter can get PD with HC group formed after brain network,the network is divided into two "default network(24,35),a hearing network(23),two of the dorsal attention network(30,32),1(4)to the top of the left frontal network,one on the right side of the forehead top network(28),2sensorimotor network(5,11),a ventral attention network(22),two visual network(6,25).Compared with HC group,the patients with PD sensorimotor network with the right amount of network,to the top of the left frontal network and enhanced the function of the default mode network connection,feeling sports network(11)and sports network(5),ventral attention network and sensorimotor,ventral attention and visual network(6,25),visual network(6)and visual(25),visual network(6,25)and auditory network,sensorimotor and auditory network,ventral pay attention to the network and the function of the dorsal attention network(32)connection.Compared with the HC group,PD-CI patients had reduced functional connectivity between the left frontal parietal network and the default mode network.Conclusion:1.In PD-CI patients,there were abnormal activation of the left orbital middle frontal gyrus,left orbital inferior frontal gyrus and right precuneus related to memory,indicating that PD accompanied by cognitive dysfunction is correlated with the functions of these brain regions.2.In PD-CI patients,the left anterior cingulate and parapingulate gyrus of brain regions with enhanced connections to the right orbital middle frontal gyrus and the right orbital middle frontal gyrus were enhanced.Changes in these connection patterns may be related to the occurrence of PD accompanied by cognitive impairment.3.In patients with PD-CI,the functional connection between the left frontal parietal network and the default mode network was weakened,indicating that the connections between cognitive-related brain networks in patients with PD with cognitive impairment were impaired to a certain extent. |
