Objective: The purpose of this paper was to detect the serum levels of adenosinedelaminate (ADA)、interferon-γ (IFN-γ) and total protein (TP) in tuberculosis pleuraland malignant pleural effusion and to evaluate these3indicators in the differentialdiagnosis and clinical significance of tuberculosis pleurisy.Methods:Eighty-nine patients with effusion hydrothorax were selected as studiedgroup. These patients were divided into two groups: the tuberculosis plevritis group(49cases) and malignant pleural effusion group (40cases). The levels of ADA、IFN-γand TP were assayed by using enzyme linked immunosorbent assay (ELISA).Results:(1)the serum levels of ADA、IFN-γ in the tuberculosis Plevritis group weresignificantly higher than those in the malignant pleural effusion group (p<0.05). Therewas no significant difference in the levels of TP between the tuberculosis Plevritisgroup and malignant pleural effusion group (P>0.05)(.2)Analysis result of ROC: theoptimal number of ADA in diagnosis for tuberculosis pleural effusion was24.5U/L,sensitivity93.9%and specificity85%; If58.19pg/ml was taken as critical value todiagnose of tuberculosis pleural effusion. The sensitivity was83%and the specificity97.5%. The critical value of TP in diagnosis of tuberculosis pleural effusion was44.4g/L,the sensitivity was81.6%and the specificity was67.5%. The areas under theROC curve of these3indicators (ADA、IFN-γ、TP) were0.93、0.98、0.58. The95%confidence internal were(0.882-0.983)、(0.960-1.000)、(0.465-0.704.Conclusion: Detecting the level of ADA、IFN-γ in hydrothorax has preferable valuein clinical application in the differential diagnosis of tuberculosis and malignantpleural effusions. Both of them can serve as an effective diagnosis index oftuberculosis pleural.
