Abstract/Summary:
Objectives Based on the different risk stratification of patients with essentialhypertension (eh) and healthy controls between plasma BNP values and24-hour dynamicelectrocardiogram records the heart rate variability index analysis of time domain, theindex changes of different risk stratification in hypertensive group, analysis of high bloodpressure risk stratification groups each plasma BNP level change and the change of heartrate variability time domain index, as well as further on plasma BNP of100pg/ml onpatients with high blood pressure were analyzed, and look into the correlation of the bothof the elevated plasma BNP and fell HRV time domain index in high blood pressure riskstratification, in order to understand the relationship between the BNP and the autonomicnervous system activity, and provide the basis for early clinical intervention.Methods In June2012to June2013collected in yan’an university affiliatedhospital dongguan branch of cardiology ward290cases of patients with hypertensionwas in the hospital, according to the latest version of2010"China hypertensionprevention and treatment guidelines"(hereinafter referred to as the "guidelines") in thepatients with high blood pressure on cardiovascular risk stratification standard will bedivided into low risk group, the moderate group, high-risk group and high-risk group offour groups, choose healthy control group20cases at the same time, the determination ofplasma BNP values, use24hours dynamic electrocardiogram, heart rate variability intime domain is (SDNN SDANN PNN50), a pair of high blood pressure, compare theresults of different risk group and healthy control group, as well as to the BNP of100pg/ml or patients with high blood pressure for the BNP and HRV time domain correlationstudies. Results1. High blood pressure plasma BNP values in low-risk group there wasno increase than the healthy comparison group, and no statistically significant (P>0.05);High blood pressure and high risk, extremely high-risk group, respectively, and the healthin plasma BNP a significant rise in value, there was statistically significant (P <0.05);And it is concluded that as the rise of the level of risk stratification, BNP value alsoincreases, there was statistically significant (P <0.05).2. Patients with high bloodpressure risk from low to the high risk group of overall HRV is reduced, to reduce thesize of the increase with layered degree aggravating. SDNN and PNN50in low-riskgroup, moderate and high-risk group, in a very high risk group were lower than in thenormal group, and there was statistically significant (P <0.05); SDANN in moderate andhigh-risk group, in a very high risk group were significantly lower in the normalcomparison group, and there was statistically significant (P <0.05); And the same timeswith the higher level of high blood pressure risk stratification, SDNN, SDANN, PNN50,in turn, reduce the trend,and there was statistically significant (P <0.05).3. In the BNP of100pg/ml or high blood pressure patients, There was correlation between the BNP andSDNN, has statistical significance (P <0.05); There was no correlation between theBNP and SDANN, and no statistically significant (P>0.05); There was correlationbetween BNP and PNN50, and there was statistically significant (P <0.05).Conclusions1. Plasma BNP values in every risk stratification of patients withhigh blood pressure in the group than the control group increased significantly, and withhierarchical levels of the rise and rise. The BNP may be hypertension predictors of riskstratification.2. Hypertension patients with heart rate variability index down, comparedwith healthy controls, and increased with the layered decreases obviously, promptautonomic nerve damage, and parasympathetic nerve from low to very high risk grouptension by increased gradually; Sympathetic nerve from dangerous groups began todecline, as tension in progressive decrease. Parasympathetic relatively sympatheticdamages for early, but slower.3. Very high risk group of plasma BNP levels inhypertension patients and there is a certain correlation between HRV time domain index.May further promote the understanding of high blood pressure patients, the clinical significance of the cardiovascular risk stratification early diet, drug intervention, delaythe time of the hypertension patient target-organ damage, so as to reduce the incidence ofhypertension patients of cardiovascular events.
