Influence Of Modified Sclerotherapy On Curative Effect And Complications Of Esophagus Varicosity Hemorrhage

Objective: Retrospective analysis of the influence useing different endoscopicsclerotherapy to the curative effect and complications of cirrhosis esophageal varicesbleeding. Comparing of the bleeding rate, complications and mortality related tocomplications, obtain a modified sclerotherapy with safer and better treatment effect.Which can be reference for clinical research.Methods: According to different endoscopic sclerotherapy in Shaanxi provincialpeople’s hospital of department of gastroenterology, during20years, patients are dividedinto two groups. Control group (traditional sclerotherapy, intravenous+by veininjection),which collected258cases of cirrhosis esophageal varices bleeding receivedour traditional endoscopic treatment from1992to1998; Improved group (intravenousinjection) was to collect268cases of patients accepted modified sclerotherapy treat from2009to2012in our hospital. The bleeding rate, complications and mortality related tocomplications are compared among the two groups.Results:(1)The hemostatic rate of improved group (intravenous injection) was98.5%, in themeanwhile, the hemostatic rate of control group (intravenous+beside intravenousinjection) was97.6%. in both groups using the χ2test, P>0.05no significant difference.(2)The incidence of complications of the improvement group was17.2%.Nevertheless,the incidence of complications was43.4%in the controlgroup,which was obviously higher than of the improved group,with statisticalsignificance.Conclusion: Endoscopic hardener treatment of cirrhosis esophageal varices bleeding is safe, effective, and reaveling higher hemostatic rate. But sclerotherapy withthe traditional method (intravenous+beside intravenous) treatment of cirrhosisesophageal varices bleeding turned up higher incidence of complications (such as fever,esophageal ulcers, esophageal stricture),as well as the mortality related to complications.Shaanxi Provincial People’s Hospital of Department of Gastroenterology improved thesclerotherapy method, which using rapid intravenous injection sclerotherapy.Sclerotherapy by the improved method does not reduce the relatively high bleeding rate.Base on that, incidence of complications (such as fever, esophageal ulcers, esophagealstricture) are reduced markedly. Which makes endoscopic hardener treatment ofcirrhosis esophageal varices bleeding reavel more operational maturity, higher safety,lesscomplications after technology improvement.
