Clinical Study Of Shoulder Hemiarthroplasty For The Treatment Of Proximal Humeral Comminuted Fractures In The Elderly

[Background]The proximal humeral fracture is a common type of fractures for theupper limbs,especially in the elderly.Most of the elderly areosteoporosis and the fractures are comminuted.The treatment ofcomminuted proximal humeral fractures in the elderly have been withouta unified strategy.Conservative treatment is applicated gradually reducedue to poor shoulder function and residual shoulder joint pain.The humerushead ischemic necrosis rate is higher of internal locking plate fixationin the treatment of proximal humeral comminuted fracture.Such fracturesif not handled properly, will inevitably lead to late in patients withchronic pain symptoms, your activity is affected by the great of shoulder,damage the body’s daily as well as the loss of function.In the elderlypatients, using half a shoulder joint replacement surgery can shoulderjoint line early functional exercise and painless, clinical applicationis more and more in recent years. [Objectives]To explore the clinical effect for the treatment of artificial humerushead replacement for humerus proximal comminuted fracture.[Methods]Analyzed retrospectively from2007.2to2013.2diagnosed with proximalhumeral comminuted fracture of the16elderly patients (Neer three andpart four fractures) and diagnosis of simple fractures (Neer second partfracture) of25of the elderly patients.The comminuted fracture weretreated with shoulder hemiarthroplasty,but the simple fracture wereperformed open reduction and locking plate fixation.Both groups werefollowed up for6to12months.Comparative analysis of two groups ofoperation time, intraoperative blood loss, Neer function score, painscore, the same shoulder joint active movement, fine rate, incidence ofcomplications of shoulder joint function.Using SPSS19.0for dataanalysis, counting data using T test, measurement data using test.[Results]Two groups in operative time, Neer function score, pain score, stretch,outside and inner spiral activity after shoulder joint, shoulder jointfunction was there was no statistically significant difference,complication rates, etc.But lift on two groups of shoulder joint pronenessand lift on outreach difference was statistically significant.[Conclusion]The recent effect is curative of shoulder hemiarthroplasty for the treatment of proximal humerus comminuted fracture in the elderly.
