Lateral accessory ligament injury of ankle is common, which accounts for about90%of ankle ligament injury. While under strong internal rotation forces, ligament causecontusions, partial and complete rupture, and its clinical manifestations is swelling, pain oflateral ankle. If early treatment is not appropriate, about10%~30%develop into chronicankle instability, reinjury, and traumatic osteoarthritis. Therefore, early definitivediagnosis and correct treatment is particularly important.Traditional imaging method of diagnosising ligament injury includes X-ray, CT, MRI.MRI is difficult to get the ideal image because of ankle position and ligament direction,and cannot be dynamic observation, expensive, not as a routine examination. Using highfrequency ultrasound can not only clearly show all parts of the lateral accessory ligamentstructure and direction, confirm the range, type and degree of ligament injury, but asloevaluate the function of the ligaments. It provides a new and effective imaging method forclinician.In this study, the patients of lateral accessory ligament injury of ankle were observedwith high frequency ultrasound, and divided into three type (Ⅰ, Ⅱ, Ⅲ type) according toultrasonography imaging, and provided objective basis to choose different treatmentmethods (conservative and surgical treatment) for clinician. In different periods, injuryligament after treatment were graded with high frequency ultrasound dynamicallyobservation by contrasting image and evaluated therapeutic effection.Contrast enhanced ultrasound is applied in wide range now, but is seldom reportedin ligament injury. while diagnosising lateral accessory ligament injury of the ankle in the early stages,color doppler flow imaging display bar blood signal, then we assume todiagnosis ligament injury with contrast enhanced ultrasound. This study was to exploreapplication value of lateral accessory ligament injury of the ankle in the early stages.Objective With high frequency ultrasonography, to evaluate ultraphonic classificationand treatment effects of lateral accessory ligament injury of ankle.Method Using PHILIP HD11, IU22and ALOKA α10color doppler ultrasonicdiagnostic apparatus, linear array probe of7~12MHz.30cases of healthy controls,148cases of ligament injury group were observed:1. The health control group: measuring thethickness of lateral accessory ligament, and make contrast of men and women, left andright side.2. Ligament injury group: injury ligament were divided into three type (Ⅰ, ⅡⅢ type), and compared with MRI diagnosis and surgical diagnosis; Ligament injury weredivided into two group:conservative or operative treatment.8cases of lateral accessoryligament injury of the ankle in the early stages were observed with contrast enhancedultrasound.Result1. There was no statistically difference of the thickness between male and female.There was no ankle statistically difference of the thickness between right and left, p>0.05.2. In conservative treatment group, there was no statistically difference of the resultbetween ultrasonic diagnosis results and MRI diagnosis results. In surgical treatmentgroup,there was no statistically difference of the result between ultrasonic diagnosisresults and surgical diagnosis results.3. Clinical Ⅰ degrees ligament injury should be conservative treatment; Ⅱ degrees andⅢ ligament damage suggest surgical traeatment.3. About20-30s afteer injecting contrast agent,bar continuous, slow blood flow wasdisplayed, and last about5min.Conclusion 1. The ankle ligaments were obviously displayed at particular sectional planes.There wasno statistically difference of the thickness between male and female. There was no anklestatistically difference of the thickness between right and left.2. The range,type and degree of injury ligament were ensured with high-frequencyultrasound. In conservative treatment group, there was no statistically difference of theresult between ultrasonic diagnosis results and MRI diagnosis results. In surgical treatmentgroup,there was no statistically difference of the result between ultrasonic diagnosisresults and surgical diagnosis results.3. To obersver the patients of lateral accessory ligament injury of the ankle,Clinical Ⅰdegrees ligament injury should be conservative treatment; Ⅱ degrees and Ⅲ ligamentdamage suggest surgical traeatment.
