| ObjectiveTo find through comparsion and analysis of the ultrasound image characteristicsof Color Doppler different molecular subtypes of invasive breast cancer, for breastcancer prognosis of preoperative assessment and individualized treatment andnon-operative therapy efficacy monitoring provide valuable imaging information.Method502cases of invasive breast cancer were divided into3groups, the youth groupunder35years old, the middle-aged group36~64years old, the elderly group≥65years, underwent two-dimensional and CDFI ultrasonic examination, observation oftumor size, L/T ratio, morphous, margin, border, hyperechoic echogenic halo, internaland posterior echo, calcification, bloodstream grade and lymph node. After measuringER, PR, HER2and Ki-67value for molecular typing: Luminal A subtypeã€Luminal Bsubtypeã€Her2/neu subtypeã€Basal-like subtype.Differences in different molecularsubtypes of analysis organization of4different types with age, histological grade andlymph node metastasis rate and correlation between the ultrasound imagecharacteristics.Result1. Ultrasound in the diagnosis of invasive breast cancer rate of accuracy was up to99%, suggesting that the lymph node metastasis sensitivity was81.1%, specificitywas85.5%.2. The proportion of Her2/neu,basal-like subtype patients in the young group wasgreater than in the middle-age and elderly group, the proportion of Luminal A subtypepatients in the middle-age and elderly group was higher than that of the young group,the proportion of Luminal A subtype patients in the elderly group was the maximum(P <0.0125).3. A multi-Luminal A-to histological grade was1or2, Luminal B, Her2/neu andbasal-like subtype patients with2and3grade were in the main (P <0.05), the rate of lymph node metastasis of Her2/neu subtype patients was highest, with the lowest ofLuminal A subtype patients (P <0.00714).4. The ultrasonic characteristics of different molecular subtype lesions showedunclear circumscription, indistinct margin,calcification proportion similar (P>0.05),no significant difference inbloodstream grade, and the largest lesion diameter, L/Tratio, morphous, irregular margin presented “sentus†or “feet of crabâ€or“process ofhornâ€, hyperechoic echogenic halo and calcification, internal and posterior echo werenot all the same (P <0.05, inter group comparison: the ultrasound images of P <0.0125, molecular subtypes of P <0.00714):Luminal A: mass was small(50.6%<2.0cm), L/T ratio was relatively large(61.4%≥0.7), morphology was irregular(80.7%), irregular margin presented“sentusâ€ï¼ˆ61.5%),hyperechoic echogenic halo(62.7%)was common, calcification(37.3%)or microcalcification(24.0%)were infrequent,internal echo was uniform(50.6%).Luminal B: mass was of irregular morphology(73.4%), irregular marginpresented“sentusâ€ï¼ˆ46.1%)or “feet of crab†or “process of hornâ€ï¼ˆ47.8%),hyperechoic echogenic halo(60.1%)and calcification(67.3%)or microcalcification(43.5%)were visible, heterogeneous internal echo was common(67.3%).Her2/neu: mass was large(75.0%≥2.0cm),L/T ratio was relatively smal(l64.5%<0.7),morphology was irregula(r77.6%),irregular margin presented “feet of crabâ€or “process of hornâ€ï¼ˆ67.1%) was common. Hyperechoic echogenic halo(40.8%)was rare, heterogeneous internal echo(80.3%), and calcification(73.7%)or microcalcification(57.9%)were common.Basal-like: mass was of regular morphology (49.2%), irregular marginpresented“sentusâ€ï¼ˆ47.7%)or “feet of crab†or “process of hornâ€ï¼ˆ44.6%),enhancement of posterior echo(38.5%), Hyperechoic echogenic halo(55.4%)wascommon, heterogeneous internal echo were common(66.2%), calcification(43.1%)or microcalcification(26.2%)were relatively uncommon.Conclusion1.Ultrasonography has high value in the diagnosis of invasive breast cancer and axillary and supraclavicular lymph node metastasis.2. Different molecular subtypes with age, histological grade, lymph node metastasisrate has certain correlation.3. The ultrasonic image characteristics with certain differences in different molecularsubtypes, there is a certain relationship between the ultrasonic image and molecularsubtypes. |
