【Objective】Papillary thyroid carcinoma is the most common pathological type of thyroid carcinoma, which shows well differentiation and low degree of malignancy. But there are still some recurrence, metastasis or even death from this kind of thyroid carcinoma.For the further exporation of prognostic factors in papillary thyroid carcinoma, based on the review of the related literature at home and abroad, a retrospective study on the patient of papillary thyroid cancer in Fujian Provincial Hospital were performed in this study, with the application of Cox proportional hazard model analysis at the same time.【Methods】 Take the retrospective study of papillary thyroid cancer cases confirmed by operation in Fujian Provincial Hospital from January2003to December2003, for a total of162. Follow up these cases by telephone or letters. Take the follow-up data into SPSSl7.0for statistics. Though the relevant research of literature at home and abroad and combined with clinical practice, take gender, age, tumor size, TNM stage, lymph node metastasis, tumor number,operation mode and capsular invasion as factors,and give these8factors quantitative evaluation. Using the Cox proportional hazard model analysis, then get the factors affecting the prognosis of papillary thyroid carcinoma. single factor analysis and multiple factors analysis were taken by Log-rank test and COX proportional hazard model, all the data has statistical significance with P<0.05in the difference.【Results】According to the Kpalan-Meier cumulative survival rate algorithm,5year,10year survival rates were93.8%and90.1%respectively.Single factor analysis showed: age, gender, staging, lymph nodes metastases, peration mode, capsular invasion significantly affected prognosis. In multivariate analysis, only age, lymph node metastases, stage were independent prognostic factors.【Conclusion】Age, gender, staging, lymph nodes metastases, number of tumors (single or multiple), operation mode, capsular invasion are related to prognosis of papillary thyroid carcinoma.Among them, age, TNM stage, lymph node metastasis were the main factors influencing the prognosis. Evaluation of the prognostic factors will contribute to make a more optimal selection of treatment of papillary thyroid carcinoma, which can reduce the complications and some unnecessary treatment, and increase the survival rate of patients.
