Objective By using microanatomy to observe the morphological characteristicof placenta and using immunohistochemical to investigate expression and clinicalsignificance of dysferlin and caspase-3in placenta of patients with hypertensivedisorder complicating pregnancy.Methods Samples were selected randomly,45women with normal pregnancy(as controls) and45pregnant women with hypertensive disorder complicatingpregnancy (in study group), including15women with gestational hypertension,15women with mild pre-eclampisa and15women with severe pre-eclampsia. Themorphological features of each groups was studied by hematoxylin-eosin (HE)staining. The expression of dysferlin and caspase-3in placenta were detected byusing SP method of immunohistochemistry, and the relationship between expressionof dysferlin and caspase-3in hypertensive disorder complicating pregnancy wasexplored.Results1. Observe the HE sections with light microscope, in normal placenta,the synacytiotrophoblast was localized in the superficial layer of the villus and itsnuclei were arranged regularly, structure was consistent. In hypertensive disordercomplicating pregnancy placenta, the trophoblast was increased, the obliteransendovasculitis was observed, the blood in the villus was decreased, and the fibrinoidnecrosis and syncytial knots were frequently encountered.2. Dysferlin was mainly expressed in cytomembrane of placentalsyncytiotrophoblast for each groups. The expression level of dysferlin in placentavillous of these pregnant women in the study was significantly lower than that ofthose pregnant women in the control group. Compared with control group(6.64±2.14),the expression level of dysferlin of mild pre-eclampisa(4.67±1.95) and severepre-eclampsia(2.07±1.83) were significantly decreased(P<0.05), and there was nodifference between that of gestational hypertension(5.60±1.45) and the control group.3. Caspase-3was mainly expressed in cytoplasm and part of nucleus of placentalsyncytiotrophoblast for each groups. The expression level of caspase-3in placenta villous of these pregnant women in the study was significantly higher than that ofthose pregnant women in the control group. Compared with control group(2.64±2.25),the expression level of caspase-3of gestational hypertension(5.33±1.45), mildpre-eclampisa(5.93±1.28) and severe pre-eclampsia(7.20±1.74) were significantlyincreased(P<0.05).4. The expression level of dysferlin and caspase-3in placentalsyncytiotrophoblast were correlated negatively, and the correlation coefficientr=-0.4167(P<0.001)。Conclusion1. Dysferlin was mainly expressed in cytomembrane of placentalsyncytiotrophoblast for each groups, and the expression of dysferlin in placenta ofpatients with hypertensive disorder complicating pregnancy was increased as thedisease worsened.2. Caspase-3was mainly expressed in cytoplasm and part of nucleus of placentalsyncytiotrophoblast, and the expression of caspase-3in placenta of patients withhypertensive disorder complicating pregnancy was increased as the disease worsened.3. The expression levels of dysferlin and caspase-3in placentalsyncytiotrophoblast were correlated negatively, and the interaction effects betweenthem may relate to the development and progression of hypertensive disordercomplicating pregnancy.
