Objective: To understand the main causes of death for inpatient with type2diabetesin Shannxi provincial People’s Hospital nearly30years. Horizontal comparison is madewith the consequences of the80s,90s and nearly a decade and analysis about the maindeath causes based on related factors among three stages in order to take effectivemeasures to prevent end points.Method: To investigate and review death data in detail to get useful information of560cases who died in every departments of Shannxi provincial People’s Hospital. Thecauses of death were divided into cardiovascular disease, cerebrovascular disease, renaldisease, infection, malignancy, diabetes and acute complications, then analysis thedifferences of three times and the effect of ages, sex, course and complications on causesof death of436cases nearly10years.Result: In recent30years, three causes of death successively are: cardiovasculardisease(30.2%-37%-42%),infection(27.9-17.3%-11.2%), kidney disease(9.4%-14.8%-9.9%),tumor(9.3%-6.2%-18.8%),cute complications(4.7%-14.8%-2.3%),the other(4.7%-8.6%-15.8%). Cardiovascular disease was the first causes of death at different times.Compared with the early2decades, the proportion died of infections and diabetescomplications were drastically reduced, but the proportion died of tumor was increasedsignificantly. In the number died of cancer, the incidence of the digestive tract tumor(colorectal cancer, liver cancer) is higher than others and malignant tumor of digestivetract could be early detected through screening CA199. In the findings of recent10years,the age of death of diabetic patients are mainly concentrated in60years later andcardiovascular disease was the first causes of death. There were436deaths in total during 2002-2012,the age of cardiovascular disease was the oldest and the acute complication′swas the youngest. The duration of DM patients mainly concentrated below10years,cardiovascular disease(44.7%)was the first causes of deaths and tumor(18.1%)wasfollowed. Tumor(20.7%) was the first causes in the number of the duration surpass10years. In the deaths of436diabetic in-patients, men was275cases, accounting for63.1%and women in161cases, accounting for36.9%during the nearly10years. Regardless ofgender, the first death cause is cardiovascular disease. there was no statisticalsignificance by the analysis of gender on the cause of death. Merged high blood pressureand infection respectively47.2%,45.2%in the patients who died in the last10years.Pulmonary infection ranked the first, followed by urinary infection. Conclusion: Among the proportion of direct death causes of DM, cardiovasculardisease ranked on top of the list all the time. More attention should be focused oncardiovascular events, especially for patients who more than60years of age. Cancerdeath causes was markedly higher than twenty year ago. We should screen CA199to oldpeople preferably. Because infection can not only directly lead to death, but also triggeror make other diseases get worse, the prevention and comprehensive treatment should notbe ignored to old age diabetics with a wide variety of infections. Age and durationinfluent the death of causes. At the same time of curing diabetes, all kinds ofcomplications should be attracted attention, such as high blood pressure, abnormal bloodlipid levels and infection. We can better improve the patient’s quality and reducemortality not only by controlling blood glucose, blood pressure and lipids strictly, butalso by preventing various complications and end points.
