A Retrospective Death Analysis Of Malignant Tumors Among Inhabitants Of Zhangjiajie City From2007to2009

Objective:This study aimed to understand the mortality status epidemiological characteristics and the disease burden of the deaths from malignant tumors in Zhangjiajie city, Hunan province. Therefore, we can identify the main malignant tumors, and which people could threat seriously and provide scientific basis for cancer prevention.Methods:The death data of Zhangjiajie residents from2007to2009were collected by the Zhangjiajie health bureau. In this research, mortality, age-standardized mortality, proportion, cause eliminated life expectancy, potential years of life lost(PYLL),Years of Life Lost with Premature Death(YLL),Indirect economic loss were calculated.Results:1. The malignant tumors mortality of Zhangjiajie inhabitants was61.02/100,000, and age-standardized mortality of that was49.50/100,000. Mortality (72.93/100,000) and age-standardized mortality (61.03/100,000) for males from malignant tumors were higher than those for females (48.32/100,000and38.16/100,000). The male Age-standardized mortality was1.6times than femal, the death of cancer in2007up to a minimum, and in2009reached the highest, the average growth rate was19.76%, the malignant tumors mortality of Cili County was the highest (63.15/100,000), and the Wulingyuan area was the lowest(35.56/10,000); the liver cancer, trachea bronchus and lung cancer and gastric cancer were the top3cancers in terms of the rank order of causes of death and age-standardized mortality2. The malignant tumors age-standardized PYLL and PYLL rate of Zhangjiajie inhabitants in the year of2007-2009was43,490.01person-years and9.72per1000, and in2007up to a minimum, and in2009at the highest. The age-standardized PYLL rate (11.4/1000) for males from malignant tumors were higher than those for females (8.15/1000), the malignant tumors age-standardized PYLL rate of Cili County was the highest(12.63/1000), and the Wuling yuan area was the lowest(7.10/1000). the top3cancers in terms of the rank order of causes of death and age-standardized PYLL rate were the liver cancer(2.39/1000), trachea bronchus and lung cancer(1.68/1000) and Leukemia(1.06/1000). The top three tumors with APYLL were leukemia(41.99),breast cancer(25.53) and brain malignant tumor(25.60).3. The malignant tumors age-standardized YLL and YLL/1000of Zhangjiajie residents in the year of2007-2009was41221.39person-years and7.69per1000,respectively, and in2007up to a minimum, and in2009at the highest.. The age-standardized YLL rate (10.45/1000) for males from malignant tumors were higher than those for females (7.65/1000), the malignant tumors age-standardized YLL/1000of Cili County was the highest(9.4/1000), and the Wulingyuan area was the lowest(5.58/1000). the top3cancers in terms of the age-standardized YLL/1000were the liver cancer(1.89/1000), trachea bronchus and lung cancer(1.55/1000) and gastric cancer (0.67/1000).4. The indirect economic burden of cancer were235,479,738RMB during the three years, accounting for0.37%of the total GDP for three years in the Zhangjiajie city. The loss of indirect economic burden per cancer patient were73269RMB, and in2007up to a minimum, and in2009at the highest.. The male was higher(139million) than the femal(0.96million), Cili County was the highest(128million), and the Wulingyuan area was the lowest(5million). The liver cancer(62million)、 trachea bronchus and lung cancer(42million) and gastric cancer (18million)were the top3cancers.Conclusions:1. the malignant tumor has been becoming the most common death cause in Zhangjiajie city;the male was higher than the femal;In2007, the lowest, in2009the supreme;the age-standardized mortality of Cili County was the highest, followed by Yongding area and Sangzhi County, the Wulingyuan area was the lowest; the top three causes of death due to cancer among inhabitants in Zhangjiajie City were liver cancer, trachea bronchus and lung cancer and gastric cancer, respectively.2.The malignant tumors age-standardized PYLL rate was the lowest in2007and highest in2009, Cili County was the highest, followed by Yongding area and Sangzhi County, the Wulingyuan area was the lowest. The top3cancers in terms of the age-standardized PYLL rate were the liver cancer, trachea bronchus and lung cancer and Leukemia. The top three APYLL due to cancer were leukemia, breast cancer and brain malignant tumor. 3. The YLL/1000and indirect economic burden caused by malignant tumor were was the lowest in2007and highest in2009, the male was higher than the femal. Cili County was the highest, followed by Yongding area and Sangzhi County, the Wulingyuan area was the lowest. The top3cancers in terms were the liver cancer, trachea bronchus and lung cancer and gastric cancer.
