Objective: To compare the clinical efficacy of retroperitoneal laparoscopic partialnephrectomy and open partial nephrectomy in order to make clear which technique isthe better surgical approach for the treatment of renal carcinoma.Methods: We retrospectively analysed73patients who underwent partialnephrectomy in Qinghai Province People’s Hospital,Urolgy, from March2011toJanuary2012. Average age of these patients’ were53.6years old.73cases weredivided into two groups respectively accepted two kinds of surgical approach, thelaparoscopic group (37cases) and the open group (36cases). The differences of renalpedical occlusion time, operation time, blood loss, postoperative hospital stay and theincidence of postoperative complications between the two groups were compared andanalyzed, and P <0.05was considered statistically significant.Results: The surgical approach of73patients were all successful. RLPN group andOPN group renal pedicle occlusion time was(32.10±5.71) min and (26.52±4.68)min,there was significant difference between them (P<0.05). The respective meanoperating time of the RLPN group and the OPN group was (128.35±18.38) minuteand(126.56±16.63) minute, There was no statistical significance between the twogroups (P>0.05). Duration of postoperative hospital stay was (7.53±1.33)day and(10.12±1.12)day, which was statistical significance (P<0.05).Intraoperative bloodloss was (102.41±55.87)ml and (140.30±77.02)ml, there was statistical significance between the two groups(P<0.05). Intraoperative blood transfusion rate was2.7%(1/37) and5.5%(2/36), two groups was no statistically significant difference (P <0.05).None of the patients of the two groups had postoperative complications, the twogroups have no statistical significance (P>0.05),too. Two groups of postoperativefollow-up, the time is12~34months, both groups were found no recurrence ormetastasis,1year survival rate was100%.Conclusion: As a minimally invasive surgical approach, RLPN has the advantage ofshorter operating time and faster recovery. But RLPN has longer renal pedicleclamping time due to operating conditions and technology. As experienceaccumulating, operation level continuously improving, and related surgical toolscontinuously developping. RLPN will be the main surgical approach for renalcarcinama.
