The Clinical Value Of Diagonsis And Treatment Of Hysteroscopy And Laparoscopy In Diagnosis And Treatment Of Infertility

Objective To explore the clinical value of hysteroscopy and laparoscopy in diagnosisand treatment of infertility,and to promote it application in clinical.Methods Retrospective analysis of126infertility patients who had been diagnosis andtreatment by hysteroscopy and laparoscpy.Analysis forms the ratio of various etiologiesabout infertility.Compare ultrasound and hysterosalpingography with hysteroscopy combinewith laparoscpy of diagnosis,evaluate the clinical value of hysteroscopy combine withlaparoscpy.Postoperative follow-up4to18months,observed in patients with pregnancy aftersurgery.Results In all of the patient,about55.6%with pelvic adhesions,about10.3%withendometriosis,about10.3%with polycystic ovary.In uterine cavity disease,endometrialpolyps accounted for42.1%,intrauterine adhesions accounted for22.8%,uterin malformationsaccounted for29.8%.HSG`s false-positive rate was15.9%(11/69),false-negative rate of39.3%(33/84) and a specificity of84.1%(58/69),sensitivity was60.7%(51/84), diagnosis was71.2%(109/153),missed diagnosis rate was39.3%.By hysteroscopy combine withlaparoscpy,there are108patients have pelvic disease and57patients have uterine cavitydisease.If we just use the hysteroscopy to diagnosis infertility patient,we would not found thepelvic disease of108patients.At the same time,we just use the laparoscpy,we would notknow57patients have uterine cavity disease.There have78person for follow-up,34patientsare spontaneous pregnancy after operation,8persons are pregnancy with the artifical assisted reproductive technologies.so,the total postoperative pregnancy rate is51.2%.Conclusions In the diagnostic accuracy of infertility patient, Hysteroscopy combine withlaparoscpy better than hysterosalpingography and only use of hysteroscopy orlaparoscopy.Hysteroscopy combined with laparoscopy can determine the causes of Infertilitydirectly and accuratly,It can diagnose and treatment uterine and pelvic cavity diseasesimultaneously.Hysteroscopy and laparoscopy have a higher clinical value in diagnosis andtreatment of infertility.
