ObjectiveTo establish a reference range of fetal splenic artery peak systolic velocity(SpA-PSV) in the normal singleton pregnancies (18–39weeks).And the differentchanges of SpA-PSV in the groups of fetails: fetuses with Henoglobin Bart Diseaseand fetuses without Henoglobin Bart Disease.MethodsThis study was conducted on uncomplicated singleton pregnancies with normalfetuses (control group,457fetuses) and pregnancies at risk for fetal Hb Bart disease(Experimental group,150fetuses)scheduled for cordocentesis at18to39weeks’gestation at Guangdong Women and Children Hospital were recruited into the study.The Experimental group were divided into three groups(without homozygous α-thalassemia、 homozygous α-thalassemia without hydrops、 homozygous α-thalassemia with hydrops). The Doppler measurements of SpA-PSV were performedby the sonographers with the high-resolution machine. The Experimental groupSPA-PSV was measured before cordocentesis and the final fetal diagnosis of Hb Bartdisease was based on fetal Hb typing using high-performance liquid chromatography.the relationship between GA and SpA-PSV was using linear regression analysis, thecomparison between without homozygous α-thalassemia groups and withhomozygous α-thalassemia using the Wilcoxon signed rank test. ResultsA total of457measurements were performed,ranging from17to26for eachgestational week (GA). The best regression model between GA and SpA-PSV wasobserved to be linear function with an equation as follows:SpA-PSV (cm/s)y=-2.83+1.237x(孕周)(R~2=0.997)。The table of nomogram for various percentileranges was constructed. The results show a continuous increase in SpA-PSV over theperiod from18to39weeks.The SPA PSV of the with homozygous α-thalassemiafetuses was higher than that without homozygous α-thalassemia.ConclusionThe reference range of fetal SpA-PSV from18to39weeks'gestation wasconstructed.This reference range may be a useful non-invasive tool in risk assessmentfor fetal anemia,. The SpA-PSV of the affected fetuses was higher than that of theunaffected ones。It’s important to check and measure SpA-PSV, which can identifyfetuses with Hb Bart disease and instruct the clinical treatment.
