Objective:To evaluate the safety, effectiveness, and feasibility of totally tubelesspercutaneous nephrolithotomy comparing traditional method in the treatment of renal andupper-ureteral calculi.Methods:60patients who underwent percutaneous nephrolithotomy when they werehospitalized in the urology of affiliated hospital of Hebei University during December2010toSeptember2011were chosen. Patients were randomized to either a totally tubeless approach(30patients) or control group(30patients). Patients were considered uncomplicated andsuitable for randomization at the end of the operation if there was no significant bleeding orresidual stone, and the pelvicaliceal system was intact. The incidence of operative time,postoperative hemoglobin,complications, hospital stay, pain, and stone-free rates werecompared in2groups.Results: No statistical significant differences were found with the time of operation,postoperative hemoglobin change,stone clearance rate,complication incidence between thetotally tubeless group and the control group(P<0.05).However,the totally tubeless group hadmuch more advantages in hospital stay,the extent of the postoperative pain and cost (P<0.05).Conclusions: Totally tubeless PCNL is safe、effective and economical procedure in thetreatment of renal and upper-ureteral calculi.However, the totally tubeless decision should betaken intraoperatively in the judgment of the operator.
