ObjectiveTo investigate the effect of total knee replacement (TKR) with or without synovectomy on bleeding and clinical outcome after surgery for primary osteoarthritis.MethodsA total of374patients meeting the criterion with primary osteoarthritis were included in the study. All the patients were randomly divided into group A, TKR with synovectomy, and group B, TKR without synovectomy. The drainage volume, invisible bleeding, blood transfusion rate were statistical analyzed between two groups. VAS principle and KSS principle were applied to measure the clinical outcomes.There were no statistically different of the positive rate of floating patella test between the two groups.ResultsPreoperatively,there were no statistically different clinical parameters between the two groups of patients undergoing TKR with or without synovectomy. Postoperatively, the drainage volume and invisible bleeding of group A was significantly higher than group B (P<0.05).There was no significant difference in blood transfusion rate. The postoperative pain, the KSS clinical and function score and the positive rate of floating patella test shows no significant differences between two groups.ConclusionWe concluded that synovectomy at the time of TKR will increase blood loss but does not provide any benefit to the long-term clinical outcomes.
