The Chronic Pelvic Inflammatory Disease(CPID) was a state of continuous chronic inflameemation in the late of pelvic infection.The major clinical manifestations were infertility, tubal pregnancy, chronic pelvic pain, which were often recurrent, refractory protracted illnessAt present, Chinese medicine has the better effect on the CPID treatment according to the deffernet symptoms of patients.Hongyuan Grenules comsisted of sargentgloryvine stem, rhizoma corydalis, houttuynia, and so on, which has the better effect on the treatment of CPID with dampness-heat and stasis, but its machemism was not clear. So, this study was to investigate the anti-inflammatory and analgesic effects of Hongyuan Grenules, and its effects on CPID treatment in rats induced by drosybenzene mucilage and mixed bacteria combined with mechanic injury, in order to provide the evidence for the clinical application of Hongyuan Grenules on CPID treatment.1. The study on anti-inflammatory and analgesic effect of Hongyuan GrenulesThis part was to study the anti-inflammatory and analgesic effects of Hongyuan Granuless used the acute inflammation animal model,such as xylene-induced ear swelling, acetic acid-induced the increase of capillary permeability in mice and carrageenin-induced paw edema in rats, the chronic inflammation animal model (cotton ball-induced proliferation of granuloma in rats), the anlggesic exerpments (Acetic acid-induced writhing and hot-induced pain in mice).the results showed that the high,middle and low dosage of Hongyuan Granules could significantly reduce ear swelling in mice(P<0.05), decrease abdominal capillary permeability in mice(P<0.05), and markly inhibit the proliferation of granuloma in rats(P<0.05or P<0.01), high and middle dosage of Hongyuan Granules could significantly attenuate the paw edema in rats at2h,4h,6h after carrageenin injection (P<0.05), high dosage of Hongyuan Granules could notably alleviate pain induced by hot plate(P<0.05), and obviously reduced the writhing times in mice within10min and10min~20min and middle dosage of Hongyuan Granules could obviously reduced the writhing times in mice within10min~20min after the Acetic acid injection (P<0.01).All the results indicated that Hongyuan Granules had significant anti-inflamation effect, including against the acute-inflammation and chronical inflammation. Meantime, it also has the analgesic effect on central and peripheral pain.2. The effect and mechanism of Hongyuan Granules on CPID rats caused by drosybenzene mucilageThis part was to study the effect and mechanism of Hongyuan Granules on CPID rats caused by drosybenzene mucilage.The CPID model was induced by injecting25%hydroxybenzene mucilage (0.04ml) into unilateral uterus of female SD rat slowly. The pathologic changes of uterus,fallopian tube and ovary in rats was observed by HE staining. The content of IL-2,IL-6,TNF-a and PGE2in serum,uterus, fallopian tube and ovary were detected with the method of ELISA.The expression of NF-KBp65,Caspase-3on uterus, fallopian tube and ovary were detected with the method of immunohistochemistry.the viscosity of whole blood was also measured. The resutls showed that high, middle and low dosage of Hongyuan Granules could obviously relieve pelvic pathologic adhesion and pathological damage of uterus, fallopian tube and ovary induced by drosybenzene mucilage. High and low dosage of Hongyuan Granules could decrease blood viscosity(P<0.05or P<0.01):high and middle dosage of Hongyuan Granules could remarkable inhibit the increase of IL-2,IL-6,TNF-α and PGE2levels in serum,uterus, fallopian tube and ovary in rats injected with drosybenzene mucilage (P<0.05or P<0.01), low dosage of Hongyuan Granules could obviously decrease the contents of IL-2,IL-6in serum, IL-2,IL-6,PGE2in uterus and TNF-a in fallopian tube and ovary(P<0.05or P<0.01) compared with model rats; high and low dosage of Hongyuan Granules could significantly decreased the expression of NF-κBp65in uterus, fallopian tube and ovary(P<0.05); high dosage of Hongyuan Granules could also elevate the expression of Caspase-3in uterus, fallopian tube and ovary(P <0.05),but low dosage of Hongyuan Granules onlyremarkably increased the expression of Caspase-3in uterus. These results indicated that Hongyuan Granules has the improving effect on the CPID treatment in rats induced by drosybenzene mucilage which is related with improve the state of blood rheology, inhibition of inflammation factor expression and promote the apoptosis of inflammation cell.3. The effect and mechanism of Hongyuan Granules on CPID rats caused by mixed bacteria injection combined with mechanic injuryThis part was to observed the effect and mechanism of Hongyuan Granules on CPID rats caused by mixed bacteria injection combined with mechanic injury. The CPID model was induced by injecting mixed bacteria (0.2ml) into scratched uterine uterus of female SD rat slowly. The pathologic changes of uterus,fallopian tube and ovary in rats was observed by HE staining. The content of EL-2,IL-6,TNF-α and PGE2in serum,uterus, fallopian tube and ovary were detected with the method of ELISA.The expression of NF-κBp65,Caspase-3on uterus, fallopian tube and ovary were detected with the method of immunohistochemistry. The results showed that high,middle and low dosage of Hongyuan Granules could obviously relieve pelvic pathologic adhesion and pathological damage of uterus and fallopian tube in CPID rats, Compared with the model group, high and middle dosage of Hongyuan Granules could remarkable reduce the contents of IL-2,IL-6,TNF-a and PGE2in serum,uterus, fallopian tube and ovary of rats (P<0.05or P<0.01), low dosage of Hongyuan Granules could obviously decrease the contents of IL-2in serum, fallopian tube and ovary, also decrease the content of IL-6in serum and uterus (P<0.01); high, middle and low dosage of Hongyuan Granules could significantly decrease the expression of NF-κBp65in uterus (P<0.05or P<0.01); middle dosage of Hongyuan Granules could remarkablely increase the expression of Caspase-3in uterus(P<0.05), high,middle and low dosage of Hongyuan Granules could obviously elevate the expression of Caspase-3in fallopian tube (P<0.05or P<0.01). These results indicated that Hongyuan Granules can attenuate the pelvic pathologic alterations of CPID rats caused by mixed bacteria injection combined with mechanic injury.In summary, this study showed that Hongyuan Granules has the therapeutic effect on CPID rats, which maybe related with its anti-flammation(inhibiting the expression of imflammation fator and promoting the apoptosis of inflammation cell), analgesic action and improvement the state of blood rheology.
