Translational Medicine Based On The Concept Of Ephedra, Schisandra Pulmonary Fibrosis Effect

Objective:With the new concept of translational medicine and Ascending, Descending, Floating and Sinking Theory, screened from the ancient literature of Chinese medicine treatment of pulmonary fibrosis, observing Herba ephedrae and Fructus schisandrae effect the pulmonary fibrosis rats through the cytokines Ang-2, VEGF, TSP-1and chemokine MCP-1, MIP-la, comparing the differences in effecting the secretory function of Alveolar macrophages and Interstitial macrophage, to verify the scientific basis of Herba ephedrae and Fructus schisandrae intervention pulmonary fibrosis.Methods:(1) Theoretical Study:Searching for the ancient experience prescriptions of pulmonary fibrosis, source cuba is "Dictionary of Chinese medicine prescriptions selection literature (1999edition), the major search the ancient prescriptions for treating "Fei Wei","Fei Bi".The modern treatment prescriptions of pulmonary fibrosis source is from Chinese scientific journals Full-text Database (CNKI), the search year range is from2003to2013, keyword search for "idiopathic pulmonary fibrosis." The literature data input Excel spreadsheet, classification these traditional Chinese medicine in prescriptions, calculated proportions.(2) vivo experiments:90male rats were divided into six groups:the model, control, ephedra alone, schisandra alone, combination of ephedra and schisandra, and hydrocortisone alone groups. Using bleomycin (7mg/kg) intratracheal instillation method to establish the model of Idiopathic pulmonary fibrosis(IPF). Half animals were sacrificed in day7, the left were sacrificed in day28. Measured the serum level of Ang-2, VEGF, TSP-1, MCP-1, MIP-1α by ELISA at day7and day28.(3) vitro experiments:Cultured the AM and IM of normal rats, were given50ul,100ul,150ul,200ul of Herba ephedrae medicated serum and Fructus schisandrae medicated serum. After72hours, took400ul supernatant of AM and IM, detecting the levels of TGF-β1by ELISA.Results:(1) Theoretical study:prescription in the modern treatment of IPF commonly used tonic medicine, lung and asthma drugs, blood circulation and antipyretic drugs. Nature is mainly cold40.38%, warm35.90%, flavors mainly are sweet40.49%、bitter32.37%. acrid19.01%, and mainly to the lung meridian based. Prescription in treatment of "Fei Bi" mainly used cough and asthma medicine, followed by Jiebiao medicine, tonic medicine, antipyretic medicine. Nature is mainly cold41.04%, warm39.55%, flat15.67%. Flavors mainly are bitter33.33%, sweet30.85%, acrid23.88%, and mainly to the lung meridian based. Drug therapy in "Fei Wei", mainly are tonic medicine, followed by antipyretic medicine, expectorant drugs, cough and asthma medicine. Nature is mainly cold38.62%, flat30.98%, warm28.49%. Flavors are mainly sweet39.08%, bitter33.62%, acrid17.87%, and mainly to the lung meridian based. By the above results and the theory of lifting, lowering, floating and sinking, screened ephedra, Schisandra. (2) in vivo experiments, at7days after modeling, the serum levels of VEGF, TSP-1in model group have significantly higher than the control group(P<0.05). The levels of chemokine MCP-1, MIP-la also have significantly higher than the control group(P<0.05). Each treatment group serum levels of Ang-2, VEGF, MCP-1, MIP-1α increased compared with the control group. At28days after modeling, the serum levels of Ang-2,VEGF, MCP-1,MIP-1a in model group have significantly higher than the control group(P<0.05). Each treatment group serum levels of Ang-2, VEGF, TSP-1, MCP-1, MIP-1α increased compared with the control group. The serum levels of Ang-2, VEGF, MCP-1, MIP-1a in treatment groups were lower than in model group, the level of TSP-1in treatment groups was higher than in model group.(3) in vitro experiments, AM,IM secrete TGF-β1levels in ephedra group and Schisandra group lower than the control group. In AM, with an increase of serum concentrations, the degree of reduction of TGF-β1level in ephedra group is higher than in schisandra group. However, in IM, with an increase of serum concentrations, the degree of reduction of TGF-β1level in ephedra group is lower than in schisandra group.Conclusions:Theory of translational medicine as a guide, from the clinical literature, we found that pulmonary fibrosis could treatment by stage, the pre-treatment is similar to the treatment of "Fei Bi", the post-treatment is similar to the treatment of "Fei Wei". Early treatment are mainly eliminating evil, tonifying qi, releasing lungs. The late treatment are mainly tonifying lung. Experimental study found that ephedra, Schisandra intervention pulmonary fibrosis may be due to its inhibition of angiogenic factor Ang-2, VEGF and inflammatory chemokine MCP-1, MIP-1α relevant. Relatively speaking, the influence of ephedra on AM is larger, the influence of fructus schisandrae on IM is larger.By transforming medicine can be a good idea to connect the experimental research and clinical treatment, so as to form a virtuous cycle of innovation to accelerate the actual research.
