Discuss the influence of Jinwu Jiangu Decoction to collagen induced arthritis rats in toes the degree of swellings arthritis index, synovial and cartilage pathological changes and the level of the transcription factors activating protein1.3.Analysis the clinical efficacy and action mechanism of Jinwu Jiangu Decoction treatment RA.Method:The clinical setting,60cases rheumatoid arthritis patients diagnosed used the method of1:1matched case control study was divided into Jinwu Jiangu Decoction treatment group and Yishen Juanbi Pill comparator treatment group, each is30cases. To Jinwu Jiangu Decoction and Yishen Juanbi Pill, during treatment two groups are conventional taking methotrexate+folic acid tablets, treatment12weeks. Watch the two groups of patients’ clinical symptoms and the signs. The experiment setting, divide60only Wistar female5Zhou Ling rats randomly into the blank group, the model group, Yishen Juanbi Pill comparator treatment group, Jinwu Jiangu Decoction high dose, Jinwu Jiangu Decoction middle dose and Jinwu Jiangu Decoction low dose, establish collagen.induced arthritis models in rats, screening the successful models in rates, excepting the blank group and the model group of rats, medicines were given to each group by intragastric administration once daily for4weeks Exploy the arthritis index integral method evaluation to evaluate index of arthritis, test C-Jun、C-Fos protein level by Western blot, take the knee and part of cartilage in pathology in testing, observation of the cell and blood vessel synovial hyperplasia, synovial layer and synovial lower lymphocytes and cartilage hyperplasia and destruction etc. Finally:(1) Clinical results:Jinwu Jiangu Decoction treatment group and control group Yishen Juanbi Pill effective rate are no significant difference (P>0.05). Jinwu Jiangu Decoction treatment group erythrocyte sedimentation rate, the power of gripping, morning stiffness time improvement rate are better than Yishen Juanbi Pill comparator treatment group after12week (P<0.05or P<0.01), the others each index compare are no significant differences (P>0.05).2groups laboratory index including CRP、ESR、RF before and after treatment are significant improvement (P<0.05or P<0.01). and so on had different degrees of improvement than treatment before. Jinwu Jiangu Decoction treatment group and Yishen Juanbi Pill control group each index is not statistically significant value(P>0.05).(2) The experimental results show that: Jinwu Jiangu Decoction treatment group and control group Yishen Juanbi Pill to CIA rats toes swelling degree, arthritis index, synovial and cartilage pathological changes and AP-1signaling pathways of two subunits C-Jun、C-Fos protein level expression have different degree of improvement.Conclusion:(1)Jinwu Jiangu Decoction treatment group can improve the clinical symptoms signs of RA patients and can improve blood sedimentation、C-reactive protein inflammatory index such as, and the curative effect is no obvious difference comparing to Yishen Juanbi Pill. Jinwu Jiangu Decoction treatment group the power of gripping, morning stiffness time improvement rate and joint activity are better than Yishen Juanbi Pill comparator treatment group (2) Jinwu Jiangu Decoction can reduce CIA rats joint swelling degree, arthritis index, improve joint synovial vascular proliferation, inflammatory cell infiltration and cartilage proliferation and can cut CIA rats AP-1signaling pathways of two subunits c-Jun、c-Fos protein level expression level. it may be one of the mechanisms that Jinwu Jiangu Decoction reduced CIA rat model of synovitis.
