Objective:To research the changes of cognitive function and the related factors in male patients with gout in coastal areas of the Province of Shandong and to provide theoretical bases for prevention and treatment of cognitive dysfunction (MCI) in patients with gout.Methods:Male patients with gout, a total of366cases were selected from gout clinic and endocrine clinic of the affiliated hospital of medical collegehe Qingdao university in the Province of Shandong in May2011to October2012; Normal control group, a total of275cases are collected from the examination center of the affiliated hospital of medical college Qingdao university and the hospital of Qingdao university and a community service center in Qingdao in the Province of Shandong. Two groups matching in age, gender, level of education, does not exist cognitive dysfunction.Record the following several conditions of the objects of this study:1. personal basic situation:gender, age, height, weight, body mass index (BMI), waist circumference, hip circumference, blood pressure, level of education.2. the general clinical data:1) past medical history:presence of hypertension, diabetes, cerebrovascular disease, cardiovascular disease, kidney disease history, etc.2) personal history:smoking, drinking, recently the number of year, etc.3) family history:presence of gout, family history of dementia.3. Cognitive function assessment:scale simple mental state examination (MMSE), Montreal cognitive assessment (MoCA), adult intelligence scale (WAIS) and HADS were used.4. Laboratory indexes:blood glucose (Glu), triglyceride (TG), cholesterol (TC), urea nitrogen (BUN), creatinine (Cr), alanine aminotransferase (ALT), aspertate aminotransferase (AST), uric acid (UA) were detected.Results:(1) There was no statistical difference of the age, body mass index (BMI), waist-to-hip ratio (WHR), education years between Gout group and normal control group (P>0.05), but systolic blood pressure (SBP), diastolic blood pressure (DBP) of gout group were significantly higher than that of normal control group (P<0.05)).(2) The levels of TG, Cr and UA, AST,rate of high blood pressure, kidney disease, smoking index were significantly higher than that of normal control group (P<0.05)(3) There was a statistical difference of MMSE and MoCA scores between Gout group and normal control group (P<0.05), There was no statistical difference of WAIS and HADS scores between this two groups(P>0.05); There were statistical differences of components of MoCA scale (visual spatial ability to execute, attention, language, and delay memory score) between Gout group and normal control group (P<0.05),(4) There was no statistical difference of MMSE and MoCA scores between patients with gout stone and patients without gout stone (P>0.05); There was no statistical difference of MMSE and MoCA scores between patients with application of drop uric acid drugs and patients without application of drop uric acid drugs (P>0.05); There was no statistical difference of MMSE and MoCA scores between patients with different seizure frequency of a year and different courses of gout(P>0.05);5) There were statistical differences of MMSE scores between gout patients,persons with high uric acid and persons with normal uric acid in the normal control group(P<0.001), MMSE scores is lower in gout patients than persons with high uric acid and persons with normal uric acid(P<0.001),(6) correlation analysis showed that MMSE scores in gout patients are negatively correlated with age (r=0.115, P<0.001), uric acid (r=0.091, P<0.001), hypertension (r=0.083, P=0.083), cerebrovascular disease (r=0.095, P=0.095) and positively correlated with the cultural level (r=0.232, P<0.001); correlation analysis showed that MoCA scores in gout patients are negatively correlated with age (r=0.145, P<0.001) and positively correlated with the cultural level (r=0.140, P<0.001).Conclusion:In this study, The Han male patients with gout in coastal areas of the Province of Shandong were selected as independent research group, the relationship between gout and cognitive function was firstly reported in China till now.1. Patients with gout have impaired cognitive function, the impaired cognitive function is associated with high uric acid, dyslipidemia, kidney disease and high blood pressure.2. Compared to persons with high uric acid in the normal control group, patients with gout still have significantly lower cognitive function, this points out that there are other risk factors to cognitive function in patients with gout in addition to long-term continued hyperuricemia.This study provided important bases for screening, early prevention and clinical treatment of cognitive dysfunction (MCI) in patients with gout and also laid a foundation for analysis of connecting gout to MCI in Chinese Han population for the future.
