[background]Acute pancreatitis (AP) is a common surgical acute abdomen, Including15-20%can develop severe acute pancreatitis (SAP). Severe acute pancreatitis is a systemic disease involving multiple organs in, Onset nasty, progress quickly, the condition is complex, much complications, high case fatality rate. In recent years the incidence of SAP has increased, easy to early concurrent multiple organ failure(MOF), One of the most common is in severe acute pancreatitis with lung injury. Accumulated evidences suggested that activated Polymorphonuclear granulocyte(PMN) that released various Pro-inflammatory cytokines, Play a pivotal role in acute pancreatities. Previous study have shown that tissue injury is PMN-dependent and protease and oxygen-radical-mediated in SAP. Study in SAP, much by chemotactic function of PMN pathogenic process mediated by cytokines. Cyclophilln A,(CyPA) is A kind of chemotactic function of protein, Is a newly discovered important PMN chemokines, CD147as its signal pathway of membrane receptor. Sepsis and some inflammatory disorders, serum CyPA expression level increased significantly, and the severity of the disease or disease activity were positively correlated. CyPA and CD147interaction on the surface of the effector cells, regulating the inflammatory reaction and development. But CyPA in SAP and its effect in the development of target organ damage is unclear.[Objective]Discussing the meaning of serum CyPA expression in SAP, analysis of its relationship with tissue damage and PMN infiltration, the determination of serum CyPA in assessing lung injury severity and prognosis of severe acute pancreatitis.[Methods]Healthy adult female Sprague-Dawley (SD) rats54were randomly divided into control group (SO group), mild pancreatitis group (MAP group) and severe acute pancreatitis (SAP group) group only18. Control group (SO group):gently turn the duodenum and pancreas after open, closed abdomen; mild acute pancreatitis group (MAP group):with0.5%taurocholic acid sodium (1.0mL/kg), it is injection, inducing the MAP model; severe acute pancreatitis (SAP group):with5%taurocholic acid sodium (1.0ml/kg), it is injection, SAP model was induced. Groups, respectively, in3h,6h,12h after random death rats, collect serum, pancreas and lung tissue specimens. Used to measure the activity of amylase (AMS) and serum ELISA method of determination of serum CyPA level; pancreas and lung tissue pathology biopsy pathology observation and evaluation, spectrophotometer in pancreas and lung tissue myeloperoxidase (MPO) activity, analysis of CyPA and inflammation of the pancreas and lung injury severity and PMN infiltrates the correlation.[Result]1. Compared with group SO, MAP and SAP group significantly increased the level of serum AMS at each time point, the pancreas injury pathology score increased significantly (P<0.01). Each time point compared with MAP group, SAP group serum activity of AMS, pancreas injury and pathological lung injury score were significantly increased (P<0.01). SO pathology group and MAP lung injury score had no significant difference (P>0.05). Therefore, using different concentrations of taurocholic acid sodium injection it is successfully established the rat model with SAP with lung injury and the MAP.2. Compared with.so, MAP group, SAP group serum CyPA level at each time point and the severity of pancreas injury are related (3h, ry=0.836, p<0.01;6h:rs=0.783, p<0.01;12h:rs=0.871, p<0.01). Compared with.so, MAP group, SAP group serum CyPA level at each time point with the severity of the injury of lung tissue is related (3h, rs=0.893, p<0.01;6h:rs=0.926, p<0.01;12h:rs=0.827, p<0.01).3. Compared with.so, MAP group, SAP group of each time point of the pancreas and lung tissue MPO activity increased significantly (P<0.01). Serum level of CyPA and pancreas and lung tissue MPO activity were positively correlated (pancreas:3hr=0.820, p<0.01,6hr=0.872, p<0.01,12hr=0.918, p<0.01; lung:3hr=0.839, p<0.01,6hr=0.759, p<0.01,12hr=0.841, p<0.01).[Conclusions]Serum CyPA expression increased in SAP, SAP can be used as evaluation severity, prognosis and early indicators of lung tissue damage.
