1ObjectiveResearch Chinese medicine preparation—Tang Bi Granule with the efficacy ofsupplementing qi and nourishing yin, promoting blood circulation to remove meridianobstruction on the influence of nerve growth factor (NGF), substance P (SP) and thesciatic nerve tissue morphology of diabetes mellitus (DM) rats, to explore theprevention and control mechanism of Tang Bi Granule to peripheral neuropathy ofDM rats.2Methods28male SD rats randomly selected were injected singlely on the left lowerabdomen with streptozotocin (STZ) according to50mg/kg dose to induce DM ratsmodel.24rats built successfully were randomly divided into model group (12),Chinese medicine group (12), normal group (12). Chinese medicine group rats feedTang Bi Granule by1.2g.kg-1.d-1and the other groups fed the same volume ratio ofphysiological saline once a day for eight weeks in a row,as measuring blood sugar,body weight and Tail flick latency(TFL) every2weeks. At the end of the experiment,rats was detected NGF and SP content in serum, expression quantity change of NGFand SP in the sciatic nerve tissue and HE staining were observed under electronmicroscope that the DM rats sciatic nerve fiber pathomorphism change, to observeDM rats sciatic nerve fiber ultrastructure under transmission electron microscopy byuranium acetate–lead citrate double staining.3Results(1)Body weight of groups of rats compared with those before treatment had nosignificant differences (P>0.05), significantly increased after8weeks treatment(P<0.01); Body weight of model group rats and Chinese medicine group rats after2,4,8weeks treatment were significantly lower than the same period of normal group(P<0.01); Body weight of Chinese medicine group rats compared with the same period of model group rats had no significant difference (P>0.05).(2) Compared with the same period of normal group rats, the fasting plasmaglucose values of the other groups were significantly increased before treatment andafter2,4,8weeks treatment (P<0.01); Compared with the same period of the modelgroup rats fasting plasma glucose values of Chinese medicine group significantlydecreased after8weeks treatment (P<0.05), thus there was no significant difference(P>0.05) after2,4weeks treatment; Compared with those before treatment, fastingplasma glucose values of Chinese medicine group rats decreased after8weekstreatment (P<0.05), but the other groups rats before and after treatment had nosignificant difference (P>0.05).(3)TFL of groups had no obvious differences (P>0.05) before treatment and after2weeks treatment; TFL of model group rats and the Chinese medicine group rats after4,8weeks treatment were significantly prolonged compared with those beforetreatment and the same period of normal group rats (P<0.01); Compared with thesame period of model group rats, TFL of Chinese medicine group rats after4,8weekstreatment were shorten (P<0.05).(4)Compared with normal group rats, NGF and SP contents in serum of the othergroups rats were significantly decreased (P<0.01); Compared with model group rats,NGF and SP contents in serum of Chinese medicine group rats were increased (P<0.05). Immunohistochemical detection displayed that NGF and SP positiveexpressions in axon and myelin sheath of sciatic nerve fiber in normal group rats weremuch, but very weak in model group rats. the Average optical density (AOD) ofmodel group rats was obviously lower than that in normal group (P<0.01); Comparedwith the model group rats, NGF and SP positive expressions in axon and myelinsheath of sciatic nerve fiber in Chinese medicine group rats were raised and the AODof Chinese medicine group rats was higher than the model group rats (P<0.05), butlower than that in normal group (P<0.05). (5)HE staining observed under electron microscope showed that: Sciatic nervefibers were full, as arranged neatly and uniformly in normal group rats; Sciatic nervefibers were disordered and loose in model group rats; sciatic nerve fiber wasneat,dense, full and uniform in Chinese medicine group rats which were closed to thenormal group.Tem observations showed that: Sciatic nerve fiber morphological of thenormal group rats was neat and myelin sheath presented plate layer structure,microfilament, microtubule and other organelles in nerve axons; Sciatic nerve fibermyelin sheath structure of model group rats were seriously loose and disordered asplate layer separated, and presented grain appearance lesions and honeycombstructure, even segmentally demyelinated. Axon was atrophied and degenerated, andpart of organelles showed uncleared; Sciatic nerve fiber myelin sheath lesions ofChinese medicine group rats significantly reduced compared with those model groupand axons slightly declined, meanwhile, microfilament and microtubule structure canbe seen. The myelin separation phenomenon was slightly degenerated.4ConclusionTang Bi Granule has the prevention and treatment function on DM ratsperipheral neuropathy, the mechanism of which may be through lowering blood sugar,increasing NGF and SP content in serum of DM rats and positive expression of NGFand SP in sciatic nerve fiber of rats, improving nerve microcirculation, and alleviatingnerve ischemia hypoxia, thus alleviating peripheral nerve injury and repairingdamaged neurons, and improving the peripheral neuropathy symptoms so as toachieve the prevention and treatment of DM rat peripheral neuropathy.
