| Objective To sum up clinical diagnosis and treatment experience of complicated biliary calculi (CBC) in order to improve the quality of the patients and lower stone recurrence raceMethods A series of118patients with CBC treated in Provincial Hospital Affiliated to Shandong University between jul.2010and jun.2012were retrospectively reviewed and analyzed. They were divided into two groups and under-went two different therapeutic methods. Group A: only remove the stone (n=33), Group B:comprehensive treatments(n=85). Comparison of comprehensive treatment and the traditional method on the therapeutic effect of hepatolithiasis, evaluate the curative effect and to instruct clinical treatment.Results Among118patients,42were men and76were women. The male to female ratio was1:1.81. During the follow up period,107cases were found. Among them,98cases were excellent, and9cases were not good.1.The patient’s sex, age, the functions of the livers and the positions of the stone before the operation have no statistical significance (P>0.05);2.A11patients were treated with operation and the process was plain sailing. 3. Patients in group B were treated by removing the stone combined with various kinds of treatment, the effective rate of treatment was94.9%, the group A was treated with removing the stone by operative during the treatment, the effective rate was82.8%.4. The mean follow up was26months (range18to41months), and no evidence of cancerous was found during the follow up period. Postoperative complications occurred in23cases, including ascites and pleural effusion in19cases, live abscess3cases, biliary and abdominal abscess1case, whom all recovered from symptomatic treatment. Conclusions1. CBC is a disease with high recurrence rate and stone residual rate, comprehensive treatment and the traditional method can both treat this disease with satisfied effect.2. Individual and comprehensive operation is the effective choice of treatment plan, and can improve life quality of patients with long-term. |
