Objective:1.To measure the regional function of the different segments in patients with myocardial infarction by real-time three-dimensional echocardiography.2. To explore the contribution of the regional function of the different segments to the global cardiac function.Methods:20cases of heart failure patients and20cases of healthy volunteers underwent RT-3DE inspection. According to17sub-division method, all myocardial segments of the patients with MI were divided into group A(the infarction segments)、 group B(the segments adjacent to the infarction area)、group C (the segments remote from the infraction area), all myocardial segments of the healthy volunteers were also divided into group A1(the infarction segments)、group B1(the segments adjacent to the infarction area)、group C1(the segments remote from the infraction area). To obtain left ventricular capacity-time curve and describe the calculation of the left ventricle in17segment by Qlab technology(6.0). The regional end-diastolic volumes(rEDV), regional end-systolic volumes(rESV), regional fractions(rEF),the ratio of regional stroke volume to global stroke volume(rgSVRatio) of the myocardial segments in patients with MI and normal patients was also assessed by real-time three-dimensional echocardiography.Results:1.Two groups in LVEF, LVEDV, LVESV and index of shrinkage not synchronous have statistical significance(P<0.05),rEF were decreased.2.Compared to the myocardial segments of the normal patients, rEDV and rESV of the infarction segments were enlarged, rEF were decreased,(p<0.05)3.Compared to the myocardial segments of the normal patients, rEDV and rESV of segments adjacent from the infraction area were enlarged (p<0.05)4.Compared to the myocardial segments of the normal patients, rEDV and rESV of.the segments remote from the infraction area were enlarged (p<0.05),rEF of the were decreased (p>0.05)5.Compared to the myocardial segments of the normal patients, rgSVRatio of the infarction segments were decreased, rgSVRatio of the segments adjacent to the infarction area were enlarge.Conclusion:1.RT-3DE can assess the regional function of the different segments in patients with myocardial infarction exactly.2.The regional function of the myocardial segments and the segments adjacent to the infarction area were decreased, the regional function of the segments remote from the infraction area were enlarged.3.The segments remote from the infraction area play an important role in the global function.4.RT-3DE could evaluate from the changes of apical in MI patients accurately.
