Objective To explore the prognostic factors of acute-on-chronic liver failure (ACLF) caused by Hepatitis B virus.Methods Two hundred and fifty three acute-on-chronic liver failure cases caused by hepatitis B virus were divided into effective group (84cases) and ineffective group (169cases). We collected and analyzed the patients’general information, conventional laboratory examination and liver function results as well as hepatitis B virus serum indicators. Student t test was used to evaluate measurement data with normal distribution, chi-square test was used for categorical variables, then find out the independent risk factors of prognosis by Binary Logistic regression analysis, and p<0.05is considered as statistical different. The entire set of data was processed by SPSS18.0(for windows) statistical software.Results The differences of gender, serum Alanine aminotransferase(ALT), Aspartate aminotransferase(AST), pre-albumin, total cholesterol, glucose, Alpha fetoprotein (AFP), hemoglobin, platelets, Hepatitis B virus(HBV) DNA level and HBeAg result between the two groups were not statistically significant (P>0.05). The differences of age, total bilirubin, albumin, creatinine, prothrombin activity, fibrinogen and hospitalization time between the two groups were significant (P<0.05). The elder, the worse the prognosis was(x2=9.426, P<0.05). Logistic regression analysis show that total bilirubin, prothrombin activity, fibrinogen, creatinine and hospitalization time were independent risk factors of prognosis(P<0.05). Conclusion Age, total bilirubin, albumin, creatinine, prothrombin activity, fibrinogen, and hospitalization time affect the prognosis of hepatitis B patients with acute-on-chronic liver failure. Total bilirubin, prothrombin activity, creatinine, fibrinogen and hospitalization time have significant effects on the prognosis and could be the independent risk factors of prognosis of hepatitis B patients with acute-on-chronic liver failure.
