Objective:To estimate the application of domestic nanometer activated carbon in the tracing and dissection of lymph node in radical gastric cancer operation. Methods:Retrospective analysis of clinical data of60gastric cancer patients for treatment of gastrointestinal surgery at Qilu Hospital of Shandong University in May2013to November2013, all patients after endoscopic biopsy and pathology of gastric cancer were randomly divided into control groups and tracer group,28patients in the control group underwent conventional gastric cancer D2-D3surgery and32patients in the tracer group were received carbon nanoparticles suspension injection before the D2-D3peration. After the surgery looked for all the lymph nodes from fresh specimen and sent then to pathology to routine HE staining. The number, the metastasis degree and black-stained of lymph node were compared. Results:The average number of the dissectional lymph node in the tracer group (33.1±11.5)) was higher than that in the control group (21.6±7.3).The metastasis degree of lymph node in black-stained lymph node of tracer group (28.2%) was higher than that in unstained lymph node of tracer group (11.1%) and the control group (21.5%). There was752black-stained lymph nodes in the tracer group, and60pieces of black-stained lymph nodes for tiny lymph nodes, its diameter is about1to2mm, in the case of no stain is difficult to clean.60tiny lymph nodes,25to transfer, positive rate (43.2%), significantly greater than all the black-stained tracer group of positive lymph nodes (28.2%), compared two groups was statistically significant difference (P <0.05).In addition, using conventional HE staining negative lymph nodes for immunohistochemical study tracer group of12patients found to have lymph node micrometastases, micro move at a rate of37.5%; Control group5patients found to have lymph node micrometastases, micrometastases at a rate of17.8%. Besides,no increase was found in the operation time and operation complications in the tracer group. No local and systemic adverse reactions happened after the nanometer activated carbon local injection. Conclusion:Local injection of nanometer activated carbon around the tumor was important to the metastatic lymph node resection, especially to the tiny metastasis lymph resection in the radical gastroectomy.
