Objective: collecting of clinical data in our hospital for120cases over65years andolder patients with severe community-acquired pneumonia from Suzhou in recently sevenyears. Retrospective analysis of the clinical features of the elderly with severecommunity-acquired pneumonia, and to investigate risk factors affecting the prognosis ofelderly patients with severe community-acquired pneumonia, provide reference in orderto further improve the effectiveness of clinical diagnosis and reduce mortality.Methods: Collect the information of our hospital from July2006to in July2013over65years patients with severe community-acquired pneumonia. Recording the patients basicinformation accord to clinical history information, vital signs, clinical manifestations, thenumber of days of hospitalization, medical history, history of trauma and surgery cases,and related laboratory tests and imaging after admission, retrospective analysis of patientclinical data, Research data using Excel sheet finishing, using SPSS18.0software packagefor data analysis. Statistical analysis is divided into two parts, first of all possible factors(22) in univariate analysis, and then multivariate analysis. Single-factor analysis ofmeasurement data as mean±standard deviation between the two groups were comparedusing t test; count data to the number of cases between the two groups were comparedusing chi-square test, P <0.05was considered statistically significant. Multivariate analysisof the univariate analysis of P <0.05indicators into the regression model of logisticregression analysis, risk factors for further screening and mortality in elderly hospitalizedSCAP calculate OR and95%CI, with P <0.05was considered statistically significant.Results:1、single-factor analysis: the age,Invasive mechanical ventilation, renal insufficiency, hypokalemia, with underlying diseases, respiratory acidosis, multi-organ failure, COPD,cardiovascular disease, cerebrovascular disease, hyponatremia, hypoalbuminemiaComparative analysis of univariate statistical difference, P <0.05. the two groups aredifference between each other;sex, smoking history, body temperature, type2diabetes,high blood pressure, liver disease, pleural effusion, chest X-ray multi-leaf lesions,non-invasive mechanical ventilation, length of hospital stay univariate analysis were notstatistically different comparison, P>0.05.2、The results of logistic regression analysis showed that the risk factors affectingprognosis is mechanical ventilation(OR=5.870,P=0.001),with COPD underlying disease(OR=2.934, P=0.043), cerebrovascular disease (OR=2.765, P=0.047), cardiovasculardiseases (R=2.643, P=0.042) Multiple organ(OR=2.187,P=0.046),respiratory acidosis(OR=1.986, P=0.034),hypoalbuminemia (OR=1.835, P=0.0270), hyponatremia (OR=1.792, P=0.006) and the age (OR=1.074, P=0.045),P values <0.05, these nineindicators are independent risk factors for hospital mortality in elderly SCAP.Conclusion: Statistics showed that the underlying disease, mechanical ventilationand the age will effects the older senior citizens with severe pneumonia.We Should payattention to the risk factors on the prognosis of elderly patients with severe pneumonia,dynamic observation of the indicators, provide local clinical data,improve outcomes,reduce mortality and provide evidence for older patients.
