| ã€Objective】To investigate the risk factors for bearing-age women in Liyangbefore and during pregnancy based on their pre-pregnancy physic examinationresults and further explore the relation between the observed risk factors and adversepregnancy outcome.ã€Methods】Physic examination before pregnancy was carried out according tothe unified national standard on fertile women in Liyang, and related indexes wereextracted for risk status analysis. Pregnant women who have received examinationduring the first42days and women who got pregnant within one year afterexamination were followed up at their home, pregnancy outcome was determined asthe end point. The relation between risk factors and adverse pregnancy outcome wasanalyzed by Chi-squared test and logistic regression. Epidata3.02and SPSS18.0wasused for data entry and statistic analysis respectively.ã€Results】1.Top five risk factors before and during pregnancy based on2524fertile women in Liyang were passive smoking (23.5%),badness pregnancy history(22.3%) overweight or obesity(12.9%), abnormal TSH level(10.4%) and age above35yr (9.9%).2.Incidence of abnormal pregnancy based on1228available women was6.8%,and the top three types were spontaneous abortion (4.1%), premature delivery (1.0%)and therapeutic abortion or labor induction (0.9%), no birth defects happened.3.Univariate analyses showed that risk factors for abnormal pregnancy outcomewere age above35yr (χ2=22.287,P<0.001), social psychological pressure(χ2=3.991,P<0.05) and abnormal TSH(χ2=12.403,P<0.001). Multivariatelogistic regression analyses revealed that risk ratio (RR) of the above three riskfactors was4.230(95%CI:2.261,7.913),2.437(95%CI:1.145,5.186) and2.863(95%CI:1.577,5.197), respectively.4.Univariate analyses showed that risk factors for spontaneous abortion wereage above35yr (χ2=25.652,P<0.001) and abnormal TSH(χ2=4.355,P<0.05).Multivariate logistic regression analyses revealed that risk ratio (RR) of the abovetwo risk factors was5.652(95%CI:2.793,11.348) and2.483(95%CI:1.156,5.328),respectively.ã€Conclusions】The major risk factors before and during pregnancy for womenin Liyang were passive smoking, badness pregnancy history, overweight or obesityand old-age. With the prevalence of pre-pregnant examination, incidence of adversepregnancy was relative lower and the abnormal outcome mainly presented asspontaneous abortion, premature delivery and therapeutic abortion or labor induction.Old-age, abnormal TSH level and social psychological pressure were the three majorhazardous factors for abnormal pregnancy outcome, besides, old-age and abnormalTSH level were also the risk factors for spontaneous abortion. |
