Part one: to research the effect of Xiaoyao San on the quality of oocytecell and embryo and endometrial receptivity and pregnancy outcomeObjective: To observe the influence of Xiaoyao San on the quality ofoocyte cell and embryo and endometrial receptivity and pregnancy outcomeMethods: Fifty-eight infertile patients meet criteria for inclusion andexclusion criteria from Bethune International Peace Hospital from February,2010to October,2011undergoing the treatment of IVF-ET were randomlydivided into experimental group (Xiaoyao San+GnRHa/FSH/hCG) andcontrol group(GnRHa/FSH/hCG). Observe the content of days and dose of theused gonadotropin, the hormone levels of E2, LH, P on HCG day, endometrialthickness and pattern, number of oocytes and mature oocytes, fertilization rate,high quality embryos rate,pregnancy rate and complications.Results:(1) The foundation data of age, BMI, AFC, basic FSH has nostatistical significance between two groups(P>0.05).(2) Comparison of thehormone levels of E2, LH, P on HCG day between two groups: The content ofE2,LH was significantly higher in experimental group than in controlgroup(P<0.05). The level of P was no statistical significance between twogroups(P>0.05).(3) Compare of days and dose of the used gonadotropin andendometrial thickness and pattern between two groups: The time ofgonadotropin were no statistical significance between two groups(P>0.05)while the dose of gonadotropin were significantly lower in experimental groupthan in control group(P<0.05). The endometrial types were no staristicalsignificance between two groups(P>0.05), but the thickness of endometriumin experimental group is thicker than control group(P<0.05).(4) Laboratoryindexes: the number of oocyte and oocyte cleavage were no statistical significance between two groups(P>0.05),while the rate of mature oocyte,fertilization, high quality embryo were significantly higher in experimentalgroup than in control group(P<0.05).(5) The clinical pregnancy rates of thetwo groups were70%(21/30)and42.86%(14/28) respectively. the pregnancyrate in experimental group was significantly higher than in controlgroup(P<0.05).(6) Comparison of complication: in experimental group had anOHSS case and a spontaneous abortion case,in control group had a case ofspontaneous abortion, there were no statistical significance between twogroups(P>0.05).Part two: The effect of Xiaoyao San on oocyte-secreted factors includeGDF-9and BMP-15in patients undergoing IVF-ETObjective: From the consideration of oocyte-secreted factors GDF-9andBMP-15in order to interpretation the mechanisms that Xiaoyao San canimprovement of oocyte and embryo quality in IVF-ET patients.Methods: Fifty-eight infertile patients meet criteria for inclusion andexclusion criteria from Bethune International Peace Hospital fromFebruary,2010to October,2011undergoing the treatment of IVF-ET wererandomly divided into experimental group (Xiaoyao San+COH) and controlgroup(COH). On the day of oocyte retrival, leaving the patients' follicularfluid, and then using lymphocyte separation medium get the granulosa cells.The protein content of GDF-9and BMP-15were detected by western-blot.The gene expression of GDF-9and BMP-15were analyzed by RT-PCR.Results:(1) The protein content of GDF-9between two groups infollicular fluid: the average content of GDF-9in experimental group(0.783±0.098) was higher than in control group(0.588±0.039)(P<0.05).(2)The expression of GDF-9mRNA between two groups in granulosa cells: inexperimental group, the expression of GDF-9mRNA is1.310±0.903while thecontrol group is0.935±0.190. The expression of GDF-9mRNA in experimen-tal group is statistic significantly higher than control group (P<0.05).(3) Theprotein content of BMP-15between two groups in follicular fluid: the average content of BMP-15in experimental group(0.876±0.0846)was significantlyhigher than in control group(0.418±0.0875)(P<0.01).(4) The expression ofBMP-15mRNA between two groups in granulosa cells: in experimental group,the expression of BMP-15mRNA is0.990±0.095while the control group is0.670±0.334. The expression of BMP-15mRNA in experimental group isstatistic significantly higher than control group (P<0.01).Conclusions:(1) Xiaoyao San combined with controlled ovarianhypersimulation can reduce the amount of Gn, which can lighten economicburden of IVF-ET patients.(2) Xiaoyao San can improve the number ofmature oocyte, the rate of fertilization, the thickness of endometrium and highquality embryo to improve pregnancy rate.(3) Xiaoyao San can improvepregnancy rate by increase the protein content and gene expression of GDF-9and BMP-15.
