Objective:1To explore the general characteristics of benign and malignant tumors.We analyzed clinical data of patients with benign and malignant tumors of thethyroid,including clinical manifestations,signs,laboratory tests and ultrasonicexamination.2To analyse the correlation of urinary iodine content and the thyroidtumor.Methods:1We collected the urine of patients with thyroid surgery in the E.N.T.department in the Fourth Affiliated Hospital of Hebei Medical University fromFebruary2012to October2013.We selected112patients with thyroid tumorswho had pathology results after operation and collected the caseinformations.We divided them into benign tumors and malignant tumors(malignant group all for the thyroid papillary carcinoma) according to theresult of paraffin pathology.We compared the two groups with various factors,which to explore its value in differentiating benign and malignant tumor.2We selected another100cases without thyroid nodules as control.Theyhad confirmed with ultrasonic inspection after health examination from June2013in our hospital.We detected urine iodine content of the patients inbenign,malignant group and normal control group,analyzed correlationbetween tumor and urinary iodine content.Results:1(1)Male,29cases,female,83cases, the ratio of male and female is1:2.8,56patients with benign tumor,56patients with malignant tumor.The ratio ofbenign and malignant is1:1.The age of patients with benign nodules rangefrom18to72years (average51.8±1.08years),malignant nodules,from21to 66years (average43.5±1.06years),the age of malignant nodules is less thanthe benign group (P=0.001).Dysphagia, hard texture and poor activity ofnodules was15,14,20cases respectively.There was a significant difference incharacteristics between benign and malignant tumor (P <0.05).Most patientsin malignant group is rural residents,71.4%,and they eat pickled food more,compared with benign group (P=0.017).(2)In thyroid function tests,94casesof thyroid function was normal,18abnormal cases.There was no differencein the incidence of abnormal function between benign and malignantgroups,14.3%VS17.9%.P=0.607.TSH in malignant group (2.24±0.47)uIU/mL was higher than the benign group (1.74±0.26uIU/mL),(P <0.05).(3)We used ultrasonic inspection to diagnose thyroid adenoma14cases,nodular goiter16cases,nodular goiter with thyroid adenoma6cases,thyroid papillary carcinoma56cases.According to the pathologicalresults,the conformity rate of thyroid adenoma was60.9%,nodular goiter was72.72%,nodular goiter with thyroid adenoma was54.5%,and thyroid papillarycarcinoma was71.4%.The total match rate was up to84.5%.Malignantnodular in nodule boundary is not clear with irregular form,low echo and richblood supply. Calcification rate in the benign group was23.2%,calcificationrate in the malignant group was57.1%, there was a significant difference (P=0.000).2Patients in both thyroid benign and malignant tumor group were in asufficient iodine state,there was a significant difference between these twogroup (P=0.000).There was correlation between thyroid cancer and urinaryiodine value.The urinary iodine value was higher in patients with thyroidpapillary carcinoma.Conclusion:1Thyroid cancer occurs mainly in middle-aged and old women, and thethyroid papillary carcinoma usually occurred in younger;Cancer generallyhave these characteristics:with dysphagia,hard texture and poor activity ofnodules. 2Eating too much preserved food was related to the incidence ofmalignant carcinoma; Incidence of malignant carcinoma is higher than theurban to rural residents.3TSH in the normal range,The higher it is, the higher the rate of thethyroid papillary carcinoma is.4The ultrasound tip:low echo and rich blood supply are risk factors tobe malignant.Tiny calcification,especially sand calcification in nodulesimplied malignant.5The thyroid papillary carcinoma patients was higher than that of normalurinary iodine level.
