Objective:Cardiac carcinoma is a common malignant tumor in China, WHO defined the tumor which through the gastroesophageal junction region for cardiac carcinoma with it’s own anatomy, organize characteristics and clinical manifestations. At present, the main method in treating patients with cardiac carcinoma is surgical treatment, but the postoperativesurvival period is not optimistic, the5year survival rate is only about20%. Cardiac carcinoma histological type was adenocarcinoma, are notsensitive to the radiotherapy and chemotherapy. Therefore, search a new therapeutic targets, for improving the treatment effect of cardia cancer,has the important practical significance. Peroxisome proliferator activated receptor (PPARs), is a kind of nuclear receptor superfamily, which can regulates and transcription of gene expression,PPAR gamma distribution in different tissues, with the regulation of cell growth, differentiation,apoptosis and metabolism of biological function. Now that the PPAR gamma expression is consider play a important part in gastric carcinogenesis, development and prognosis. The expression of PPAR γ in gastric cancer tissues was significantly higher than the expression in normal gastric tissues, and increased with the depth of tumor invasion, lymph nodemetastasis, clinical stage increasing, decreasing tumor differentiation degree. In the relationship of PPAR γ and esophageal cancer, The expression of PPAR γ in esophageal cancer tissues was significantly declined, that decreased expression is related to Poor prognosis. PPAR γ and its ligand may also play an inhibitory role in the occurrence and developmentof esophageal cancer. But in esophageal cancer and gastric cancer, along with the increase of the depth of tumor invasion, lymph node metastasis, clinical stage, distant metastasis, the expression of PPAR γ mRNAhas the opposite result, the specific mechanism is not clear, further labo ratory research and epidemiological confirmed. At present there is no relationship between the biological characteristics of the PPAR γ and cardia cancer, This studie explore the expression of PPAR γ in gastric cardiacand the relationship between the biological characteristics and cardia cancer, to explore the possibility of PPAR γ to be a gastric cardia gene therap.Method: the specimens were collected from the fourth hospital of HebeiMedical University between2013.2-2013.1050cases of operation excision ofcardiac carcinoma, and corresponding normal tissue (8cm above the edgedistance carcinoma after operation, confirmed no residual cancer stump).Using real-time PCR and immunohistochemistry method to detect theexpression of PPAR γ in gastric cardia. Use SPSS17.0for statistical analysisof the data, Expression of PPAR γ protein was using chi square test forstatistical analysis, P<0.05with statistical significan. the expression levels ofPPAR γ mRNA by Mann Whitney U test, with P<0.05indicate significantdifference, comparison of multiple groups of data by Kruskal-Wallis H test toshow significant difference, with P<0.05indicate significant difference. Geneexpression levels were calculated using2ΔΔCt method.Result:The results of immunohistochemistry:1Positive signal of.PPAR γ protein was mainly located in the nucleusand(or)cytoplasm, positive cells in the cytoplasm and (or) the cell membraneis brown with yellow granules calm.50cases of cardiac carcinoma IHCshowed:24cases of the positive expression of PPAR γ protein in cardiaccarcinoma, the positive rate was48%,38cases of positive expression in50cases of far carcinoma tissues, the positive rate was76%. The statisticalanalysis showed that the difference between two groups was significant (χ2=8.319, P=0.004), can consider PPAR γ protein in cardiac carcinoma positiveexpression than far carcinoma tissues.2Statistical analysis showed that, PPAR γ protein expression related to lymphnode metastasis (χ2=6.611, P=0.01), related to TNM stage (χ2=8.552,P=0.014), has Irrelevant with the degree of tumor differentiation, depth of invasion, gender, age (P>0.05)Real-time PCR results1In50cases of gastric cardia, median expression of PPAR γ mRNA in cancer tissues was1.07498far carcinoma tissue expression, median expression was3.25193. The results showed that there was significant difference between the expression of cardiac cancer tissue and far carcinoma tissue (P=0.01), can consider the expression of PPARγmRNA in gastric cardia carcinoma was lower than that in far carcinoma tissue.2Expression of PPAR γ mRNA is related with the degree of differentiation,and its expression was decreased with decreasing the degree of differentiationin gastric cardia cancer, median expression of well-differentiationadenocarcinoma was1.935745moderately differentiated in expression was1.168503, poorly differentiation in expression0.977991, between the group ithave significant difference (P=0.013). It has relationship with gastric cardiacarcinoma and TNM staging, median expression amount was2.487083instage Ⅰ, the median expression of stage II was1.626207, stage III inexpression0.532558, there was significant difference (P=0.01).and relate tolymphatic metastasis, without lymphatic metastasis of gastric cardiacarcinoma in the expression of1.732496, PPAR γmRNA median expressionamount of0.62464is associated with lymph node metastasis of carcinoma ofthe gastric cardia, and there are significant differences (P=0.01). Associatedwith the depth of invasion, infiltration the mucosa or submucosa PPAR γmedian expression amount was2.151459, infiltrating the muscularis propriaPPAR γ median expression amount was2.114262, serosal layer in a PPARgamma expression amount of1.070031, infiltrated serosa layer, adjacent organinvasion PPAR γ in expression0.966161, there were significant differences(P=0.01). There was Irrelevant with age, sex (P>0.05). There were no distantmetastasis in patients which underwent radical resection with cardiac cancer,so it is unable to statistics the relationship between PPAR gamma expressionand distant metastasis.Conclusion:1The expression of PPAR γ mRNA in gastric cardia carcinoma was significantly reduce than in far carcinoma tissue, and related to tumor TNMstage, differentiation degree, invasion depth, lymph node metastasis, Irrelevantwith the gender and age.2Significantly reduce the expression of PPAR γ protein in cardiac carcinomathan in far carcinoma tissue, related to tumor lymph node metastasis, TNMstage, Irrelevant with the the degree of tumor differentiation, depth of invasion,sex, age.3The expression of PPAR γ were lower than far carcinoma tissue is at genelevel and protein level, it was not completely consistent with the biologicalcharacteristics, which may exist error. These results suggest that the reduce ofactivity in PPAR gamma may be involved in the carcinogenesis of gastriccardia carcinoma, and its antitumor activity decreased in gastric cardiacarcinoma may promote the occurrence and development of gastric cardiacancer.
