Objective: To establish experiment model of partial hepatectomy withdifferent volume liver lobe congestive in rats and investigate the effect ofresidual liver regeneration variation trend and liver function after partialhepatectomy in rats of early stage.Methods:Experiment is divided into two steps, first,To establish expe-riment model of partial hepatectomy with different volume liver lobecongestive in rats.second,study the early liver regeneration with this model.①The research object using SD rats which aged6-7weeks old, weighingbetween200-250g. According to the different volume liver lobe congestive,rats were divided into three groups. Group A:70%of liver resection group(middle+the left lobe); Group B:70%liver resection group (middle+the leftlobe) combined half of caudate lobe congest(congest volume accounting forone-sixth of the remaining liver volume);Group C:70%liver resection group(middle+the left lobe) combined the whole caudate lobe congest(congestvolume accounting for one third of the remaining liver volume); Each groupof20. The rats were sacrificed after congestion24,48,72and120h.②Specimen collection:Blood samples were collected before the rats weresacrificed and liver samples.③D etection of relevant indicators: measurementALT, AST levels in serum (to be automatic biochemical analyzer);pathological observation (with HE staining); immunohistochemical stainingabout1iver cells proliferating nuclear antigen PCNA and Ki-67. RecordPCNA and Ki-67expression;Detect hepatocyte growthfactor (HGF) and TNF-aby enzyme-linked immunosorbent assay (ELISA).④The results ofgroup A、group B and group C were analyzed using SPSS17.0statisticalanalysis software,observe comparison of various indicators of groupdifferences in liver regeneration (SNK comparative analysis) results wereexpressed as means±standard deviation, the inspection level a=0.05.Results:part1: Research and choose the stable experiment model of partialhepatectomy with different volume liver lobe congestive in rats.part2: The liver function:The experimental group and control group inthe level of serum AST, ALT is at its peak in the first postoperative day l Andwith the passage of time gradually reduced.The control group a comparedwith the experimental group B and C, ALT、AST levels were significantlyhigher than those of controls (P <0.01)in first,second,third day after surgery,after5days no statistical differences between groups.B and C had nostatistical difference.2.The right liver lobe liver ratio:Group A compared withB and C,Group A was lower than that in group B and C with the first andsecond day after surgery,(P<0.01). the third days after surgery, group A islower than B and C,(P <0.05), after the fifth day has no obvious differencebetween three groups,(P>0.05)3.serum HGF levels:The first postoperativeday3groups of rats serum HGF value reaches its peak, then gradually decline,group A compared with two groups of B, C,the HGF value significantly lowerthan the experimental group (P <0.01), while B and C no difference (P>0.05),5days between the three groups with no statistically significant (P>0.05).4.Right liver homogenate tissue TNF alpha: B, C of TNF alpha valuetwo group was obviously higher than that of group A in postoperative days1,2,3(P=0.000), B compare with C group with no difference (P>0.05)and the second day after surgery TNF alpha value reaches its peak.5days nodifference between the three groups of TNF alpha value,(P=0.217).5pathology: No visible bruising hepatic biopsy liver cell division andproliferation, the first day after the peak. The first day of congestion lobeseen spotty necrosis of liver cells, to the fifth day of complete necrosis ofliver cells has been difficult to see the normal cells.6.liver tissue PCNAindex: PCNA expression levels in liver cell nuclei of the first day wassignificantly increased in the first two days after reaching a peak, and thengradually declined; A group and B, C,2days in the postoperative phase tworatio, significantly lower than B(P<0.01), after the first3,5days, nosignificant differences among the three groups (P>0.05),7. Ki-67index ofliver tissue: the first day significantly increased, on the second day afterreaching a peak, and then gradually declined; Group A was lower than thatin group B and C in1,2,3days after surgery(P<0.05),, after the fifth day hasno obvious difference between three groups(P>0.05).Conclusions:1.Based on the congestion model of the rats after liver resection,we canprove①: Too much congestion range (1/2) or residual liver volume is toosmall (left lobe only10%) result in the liver regeneration function obviouslylimited and increased mortality.②70%liver resection with half caudate lobeor the whole caudate lobe congestion model is stable. This model can be usedto study effects on liver regeneration with congestion。2. Compare the liver damage,congestion group was significantly higherthan the group without congestion in early,indicating liver congestion leads tolocal small-scale temporary impaired liver function,Five days later returned tonormal and the other regeneration index were not statistically significant.So we can say Small range of passive congestion will eventually liverregeneration have a negative impact.but does not have a negative impact onliver regeneration. The long-term observation results show that the congestionlobe become necrosis and makes the chance of rat abdominal cavity infection,increased mortality.3. Congestion lobe may stimulate normal residual liver produce moreinflammation and liver regeneration factor,Accelerate liver regenerationprocess, but if the congested area is too big or residual liver volume is toosmall, this process may be inhibited.
