ObjectiveTo observe the onset time of hemorrhagic transformation in cerebral infarction of patients with embolism infarction and thrombosis infarction, summarize the regularity of onset time, and give a new MRI classification through sumpping up the MRI characteristics.Methods1.We collectted72patients of HT who were no thrombolysis therapy,42males and30female, and aged in30-82years old.In which included embolism infarction (22patients)and thrombosis infarction (50patients). The diagnosis if was embolism infarction or not were maded by the medical history, electrocardiogram, carotid artery ultrasound,Ultracardiography,transcranial doppler sonography (TCD) and so on.All of the images were diagnosised by two doctors engaged in imaging diagnosis for many years.2. The onset time of embolism infarction and thrombosis infarction was obtained by the progress note.We statisticsed the quantity of which happened at1-3days,4-7days and after7days, and analyzed the data by the statistical package of SPSS17.0and was thought to be significative when P<0.05. The CT images was made by Siemens Germany Siemens64layer spiral CT scan, and the MRI by the United States GE company singa CV/I1.5T.We give a new MRI classification by recording the location,forms and incidence of different location and forms of all of cases.ResultsThere were67patients who happened HT in the first week after symptom onset in cerebral infarction,and5patients happened beyond one weeks.The peak time of HT in embolism infarction (the1-3th day) is earlier than thrombosis infarction(the4-7th day)(P=0.017).HT only happened in the supratentorial were at the most (84.7%), subtentorial were second (12.5%), mixed (the supratentorial and subtentorial) were the least (2.8%).For the bleeding morphology, mottled patch sample were most (55.3%), line sample taked second place (39.4%), hematoma were the least(5.3%).At the same time.the line sample bleeding were found in the lobe mostly(89.2%), and the mottled patch sample were mostly happened in basal ganglia (48.2%). ConclusionThere are different peak of time between embolism infarction and thrombosis infarction,and the time of embolism infarction is earlier than thrombosis infarction. We combined with the position, the shape and the incidence rate of different position and shape and applied the new MRI classification as:HT1(supratentorial non-Hematoma), HT2(subtentorial non-Hematoma); HT3(hematoma), HT4(mixed non-hematoma).
