Objective:The study is to explore the risk factors of hemorrhagic transformation. To investigate the risk factors of HT in patients who are attributed to large artery atherothrombosis(LAA)or cardiogenic cerebral infarction(CE).Methods:We collected all the clinical data of 300 cases of acute ischemic cerebral infarction in the Second Affiliated Hospital of Soochow University from January 2016 to May 2018.According to whether there was hemorrhagic transformation, the cases were divided into two groups:HT group and non-HT group. In the other hand, according to TOAST classification,to divide them into five groups:large artery atherothrombosis, cardioembolism group, small artery occlusion group, stroke of other determined cause group, stroke of undetermined cause group.Baseline demographic and clinical information were collected and to analyze the risk factors of HT and different subtypes of cerebral infarction.Results:1.Most of hemorrhagic transformation occurred within one week after cerebral stroke.2.There were thirteen risk factors that were significantly different between HT and non-HT group(P<0.05),including age, atrial fibrillation, massive cerebral infarction and so on.Two factors including atrial fibrillation, massive cerebral infarction were the risk factors for hemorrhagic transformation through multivariate regression analysis.3.The subtype of cardioembolism(CE)accounted for the highest proportion in TOAST developing HT,followed by large artery atherosclerosis(LAA),small artery occlusion(SAO)was the lowest.4.Massive cerebral infarction was the independent risk factors of hemorrhagic transformation in subtypes of large artery atherosclerosis in TOAST.Conclusion:1.Most of hemorrhagic transformation occur within one week after cerebral stroke.2.The study finds that the risk factors of hemorrhagic transformation are atrial fibrillation, massive cerebral infarction.3.The cardioembolism has the highest incidence of HT in TOAST.4.The independent risk factor for subtypes of large artery atherosclerosis in TOAST developing HT is massive cerebral infarction.
