Objective: To investigate the clinical effects of VSD combined with reverseposterior forearm flaps for treatment of hand peeling-machine wound.Methods:53cases with hand peeling-machine injury were treated. For the firststage, the operations, such as hand debridement, fracture reduction and internalfixation, soft-tissue repair, the VSD placement surgery were performed; At thesecondary surgery, we took hand debridement, and use flap with a pedicle of theposterior interosseous artery to repair the wound surface. And evaluate the curativeeffect of post operation.Results: Postoperatively,53flaps survived completely.44cases of patients withwound healed uneventfully.9cases of patients with wound healed by dressing andagain debridement, flaps survived completely.43flaps follow-up of9.5months (from7months to12months), the flap texture, blood, color and texture were good, For4cases, debulking procedures were performed defatting. According to TAM scales, theresults of11cases were regarded as excellent,9cases better,9cases good and14casesthe worst, with an excellent and better rate of46.5%.Conclusion: Compared the treatment method of using flap with a pedicle of theposterior interosseous artery cover the wound surface only, combination with VSDsignificantly decrease the infectious rate of wound, improved the flap survival rateand clinical results.
