Purpose: The purpose of this paper is to through the observation ofintracranial tumor patients during perioperative application of sevoflurane,isoflurane inhalation anesthesia in patients with plasma inflammatory factordifferences, and comparison of sevoflurane and isoflurane’s influence on theability to resist infection postoperatively in patients with intracranial tumors,and on the level of cytokine reaction their effects on the body’s immunefunction, provide the basis for intracranial tumor patients to drugs in clinicalanesthesia.Methods: This experiment select elective in30patients with intracranialsupratentorial tumor resection, Male, the female (n=15), aged30-65, ASAI-class II, randomly divided into two groups namely sevoflurane group (SEV),isoflurane group (ISO),15cases in each group. Two groups of patients inpreoperative intramuscular injection atropine0.5mg, half an hour after thehouse open venous pathway, regular monitoring of blood pressure, heart rate,blood oxygen saturation, MAC value indicators, etc. General anesthesiainduction adopts the mi da azole shimron0.05mg/kg,2mg/kg propofol,fentanyl2-5mu g/kg and atracurium0.15mg/kg for inducingintubation..Respectively in two groups of patients inhaling isoflurane andsevoflurane, maintain the MAC value between1-2MAC, and intermittentfentanyl and atracurium intravenous administration to maintain anesthesia,make the BIS in40-60. Two groups of patients before anesthesia (T0),operation began after an hour (T1), two hours after the start of the operation(T2) finish (T3),24h after surgery (T4) in peripheral venous blood2ml, High-speed centrifuge for10min at low temperature (2000r/min), and thentake that0.51.0ml supernatant fluid (plasma) into EP canal, on lowtemperature refrigerator for unity in the-80°C, the final concentration inplasma by ELISA method, the TNF alpha, IL-6and the concentration of CRP.Results:1, The change of the concentration of serum TNF alpha:Plasma TNF alpha group comparison: ISO group at T1time points comparedwith T0time points in plasma TNF-a no obvious difference (P>0.05),significantly increased in T2, T3time points (compared with T0time points, P<0.05), T4time points from the previous lower but still significantly higherthan the T0time points (P <0.05). SEV group compared with T0time pointsT1time points in plasma TNF-a significantly higher (P <0.05), T2, T3, T4gradually decreased, But the T0time point is still high (P <0.05).Comparison between groups in T3time points in ISO TNF-a concentrationhigher than SEV group (P <0.05).2, The change of plasma concentration of IL-6:Group comparison: SEV and ISO group the change of the plasmaconcentration of IL-6in T1, T2and T3time points compared with T0timepoints were significantly higher (P <0.05), gradually decline in T4time points,slightly above the level of the T0, but no statistical significance (P>0.05).Comparison between groups in T3time points in SEV plasma concentrationof IL-6below ISO group (P <0.05).3, The change of plasma CRP concentration:Group comparison: Content in the T1point in time of two groups of patients inthe CRP, there was no significant difference compared with T0timepoint (P>0.05), then increased gradually, T3point in time to peak (P<0.05), at T4time point were back to the level of T0(P>0.05).Comparison between groups: T3time points, the concentration of SEV groupwas lower than that of ISO group of two patients with plasma IL-6(P<0.05).Conclusion:1. intracranial tumor patients with application of sevoflurane andisoflurane inhibits inflammatory cytokine TNF alpha and IL-6and CRP ofinflammatory mediators. The recovery of immune function in patients withtumor has positive significance.2. sevoflurane isoflurane can better inhibition of intracranial tumorpatients who shortness of plasma inflammatory cytokines IL-6and thesecretion of CRP and inflammatory mediators of TNF alpha isofluranerestraining effect is small and short, on the choice of anesthesia.
