Objective To explore the diagnosis and treatment value of fiberoptie ductoscopy (FDS) in patients with breast ductal diseasesMethods From February2010to December2012,438patients with breast disease underwent FDS and mammography angiography in our hospital, of which290patients received surgical resection and pathological diagnosis. FDS with pathological diagnosis,mammography angiography,and methylene blue injection were analyzed respectively.Results Of all438breast disease patients,397cases (90.64%) with nipple discharge,41cases (9.36%) without nipple discharge.312cases (71.23%) received FDS examination, and no abnormal findings in23cases (7.37%);25cases (8.01%) were diagnosed as ductal dilatation or inflammatory lesions;264cases (91.67%) with intraductal space-occupying lesions, of them papilloma occured in182cases (68.94%), papillomatosis in69cases (26.14%), breast ductal carcinoma in13cases (4.92%), all of them underwent surgery, the pathological detection rate was91.29%(241/264).126cases (28.77%) received mammography angiography, no abnormal in27cases (21.43%), ductal dilatation or inflammatory lesions in73cases (57.94%) and intraductal space-occupying lesions in26cases (20.63%). In26cases of intraductal space-occupying lesions, papilloma in16cases (61.54%), papillomatosis in9cases (34.62%), breast ductal carcinoma in1case (3.85%), all underwent surgery, the pathology detection rate was26.92%(7/26).201cases located by ductoscopy positioning and underwent surgical resection, the pathological detection rate was97.01%(195/201).63cases located by methylene blue injection and underwent optional section resection, the pathology detection rate was73.02%(46/63).Grouping analysis showed:FD detection results and pathology diagnosis results had high consistency (P>0.001).The FD detection group results significantly higher than mammography angiography detection group (P<0.001).The detection results of the FD positioning group was significantly higher than the methylene blue inj ection group (P<0.001)Conclusion In examinations of breast ductal diseases,FDS has obvious diagnostic accuracy and positioning reliability and it was better than mammography angiography. It was not only improved the diagnosis of intraductal space occupying lesion, determine the nature and site of lesions, but also could directly do the local treatment. So it improved the diagnostic rate of early breast cancer and had high clinical value in treatments, the guidance of breast-conserving surgery,as well as reducing the potential cancer risk in women who was at high risk.
