Objective The purpose of this study is to evaluate the preventive effect of San huang Ointment on catheter-associated phlebitis by subjective and objective evaluation tools, thus to provide scientific bases for preventing peripheral intravenous catheter-associated phlebitis.Methods105hospitalized patients receiving peripheral intravenous catheter were randomly divided into three groups:group A of San huang Ointment (n=35), group B of Hirudoid (n=35) and group C (the blank group, n=35). The patients of there groups were inserted peripheral intravenous catheter by standard procedure. In the Group A, the areas of wiping San huang Ointment were6cm long,4cm wide,3cm thick beyond0.5cm from the puncture point.Group B wiped by Hirudoid with the same area as group A immediately after infusion. Group C was only performed routine care. Subjective and objective evaluation tools are used to compare the incidence of phlebitis, the severity of phlebitis and indwelling time; To discuss the consistency of results of subjective and objective evaluation; To observe the relationship between San huang Ointment and degree of comfortness; Meanwhile, to explore the related factors of peripheral intravenous catheter-associated phlebitis.Results (1) The results of subjective evaluation:the incidence of phlebitis of3groups had significant differences (P<0.05). It showed that group A was lower than group B (P<0.05) and group C was the highest (P<0.05).(2) The results of objective evaluation:in3groups, the damaged degree of the veins punctured had significant differences (P<0.05). Compared with Group B, group A was better (P<0.05) and group C was the worst (P<0.05).(3) According to the value (0.515) of Kappa test, the results of subjective evaluation were in moderate consistency with objective evaluation (P<0.01).(4) The indwelling time of peripheral intravenous catheter had significant differences between group A and group C (P<0.05), However, if the indwelling time was more than5days, the positive rate of venous thrombosis was also statistically significant (P<0.05).(5) There were significant differences found in comfortness between group A and group B (P<0.05).(6) The occurrence of the peripheral intravenous catheter-associated phlebitis was related to the indwelling time and the number of times of punctures (P<0.05).Conclusions (1) External application of San huang Ointment early can prevent phlebitis caused by peripheral intravenous catheter.(2)Results between the subjective and objective evaluation tools had moderate consistency.(3) There is no manifest impact on prolonging indwelling time by external application of San huang Ointment; and indwelling time of peripheral intravenous catheter is safe within five days.(4)External wipe San huang Ointment can reduce the discomfort of patients caused by inserting peripheral intravenous catheter.(5) From analysis, it can prevent phlebitis for patients to provide safe indwelling time, strengthen related education and improve the consciousness of self-nursing.
