Objective:This project aims to clarify the influence of the TCM constitution to the occurrence and progress of hypertension during the special physiologic period of female by analyzing the distribution regulations of TCM constitution of the patients with PMEH and discuss the correlation of different constitutions to the classification of blood pressure, risk stratification and BMI in order to guide the women of this age group to combine the characteristics of TCM constitution with themselves to preserve their health, change the pathologic constitutions into better conditions and recover the balance of Yin and Yang of the body, avoid the risk factors of hypertension and prevent the occurrence and progress of PMEH.Method:A total of240patients who conformed to the standards of PMEH were recruited from September2011to October2012.They were outpatient or inpatient in treatment in cardiology department of Southeast hospital affiliated to Xiamen University. We made a PMEH questionnaire table referring to the standard of TCM constitution classification and judgement compiling by China Association of Chinese Medicine. Our investigators asked the patients item by item to fill the questionnaire and judged their constitution. The clinical materials of the patients are collected such as general conditions, smoking history, BMI, Bp, FBG. blood fat, blood uric acid, serum creatinine, ECG and cardiac color ultrasound. SPSS17.0software package is used for statistics and analysis. Enumeration data is analyzed by χ2-test and measurement data is showed by (x±s). The comparison between the two groups of measurement data are as follows:T test is used for the type with normal distribution and homogeneity of variance. Wilcoxon test is used for the measurement data and ranking data without normal distribution and homogeneity of variance. Variance analysis or q test is used among multi groups. The statistical significance is P<0.05.Result:1.The constituent ratio of TCM constitution type of PMEH patients is most common in yin deficiency type,phlegm-damp type and blood stasis type.63.75%of the constitution are this three biased constitutions of which the constituent ratios are separately29.17%,20.00%and14.85%. The constituent ratios of other constitution types are as follows:gentle type5.42%,qi deficiency type8.33%,yang deficiency type10.42%,damp-heat type4.17%,qi depression type6.25%and the special type1.66%. 2.The difference of the constituent ratio of Bp stratification is statistically significant (P<05) among every constitutional type of PMEH patient. The difference of the constitution is statistically significant (P<0.05) between the comparison of phlegm-damp type, damp-heat type, blood-stasis type and qi deficiency type, qi depression type, gentle type. Stage3hypertension is common in the patients of phlegm-damp type, damp-heat type and blood-stasis type. Stage2hypertension is common in the patients of yin deficiency type and yang deficiency type. Stage1hypertension is common in the patients of qi deficiency type, qi depression type and gentle type.3.The difference of the constituent ratio of hypertension risk stratification is statistically significant (P<0.05) among every constitutional type of PMEH patient. The difference of the constitution is statistically significant (P<0.05) between the comparison of phlegm-damp type, damp-heat type, blood-stasis type and qi deficiency type, gentle type. The very high risk hypertension risk stratification is common in the patients of phlegm-damp type, damp-heat type and blood-stasis type. The high and middle risk ones are common in the patients of yin deficiency type and yang deficiency type. The low risk one is common in the patients of qi deficiency type and qi depression type.4.The comparison difference of BMI is statistically significant (P<0.05) between phlegm-damp type and damp-heat type.The comparison difference of BMI is statistically significant (P<0.01) between other constitutional types with phlegm-damp type and damp-heat type.The common body shape of the PMEH patients with the constitutional type of phlegm-damp and damp-heat in this study is overweight(24≤BMI≤28) or obesity (BMI≥28).5.The difference of the constituent ratio of menopause situation is statistically significant (P<0.01) among every constitutional type of PMEH patient. The comparison difference is statistically significant (P<0.05) between the qi depression type, phlegm-damp type, damp-heat type and yin deficiency type, yang deficiency type, qi deficiency type and blood stasis type. The non-menopause people with hypertension is common in the patients of qi deficiency type, yang deficiency type, yin deficiency type and qi depression type. The post-menopause people with hypertension is common in the patients of phlegm-damp type, damp-heat type and blood stasis type.Conclusion: 1.Biased constitution is common in PMEH patients and the main pathologic constitution is yin deficiency type, phlegm-damp type and blood stasis type.2.The constitutional type of PMEH patients is closed related to Bp ranking and risk stratification. Stage1hypertension with low risk is common in the people with qi deficiency type and gentle type. Stage3hypertension with very high risk is common in the people with phlegm-damp type, damp-heat type and blood stasis type.3.The constitutional type of PMEH patients is closed related to BMI.The common body shape of the PMEH patients with the constitutional type of phlegm-damp and damp-heat is overweight or obesity.4.The TCM constitution of qi deficiency type, yang deficiency type, yin deficiency type and qi depression type is common in PMEH patients during menstrual disorder period.while the phlegm-damp type, damp-heat type and blood stasis type is common during postmenopausal period.
