Objective:To investigate Efficacy difference between the flexion and extension excise therapy in patients with non-specific low back pain.Methods:120patients with degenerative low back pain were randomly divided into Blank group,Flexion and Extension group,Blank group contain20patients and the ohers contains50. Flexion group:patients in this group take flexion exercise and Shujin Huoxue Pian; Extension group: patients in this group take extension exercise and Shujin Huoxue Pian; Blank group: patients only take Shujin Huoxue Pian.Patients in flexion and extension groups have took exercise therapy for one year in rehabilitation department of our hospital.Observed and compared items,including clinical curative effect,VAS scores for evaluating the strength of lumbar pain,ODI dysfunction questionnaire for the lumbar’s function,lumbar lateral X-ray films on the change of lumbar curvature and the change of cross-sectional area of paraspinal muscles on L4-5level by MRI before and after treatment for one year. The patients blank group follow-up included VAS scores, ODI dysfunction questionnaire, improvement rates and clinical efficacy.Results:The difference of the VAS, ODI scores in three groups was not statistically significant (all P>0.05) before treatment. Aafter treatment, the VAS and ODI scores were less than those of before treatment in each group,and the difference was statistically significant (all P<0.05). After treatment,the pairwise comparisons of VAS, ODI scores are different from each orther,and the differences were statistically significant (all P<0.05),and lower scores in extension group are most obvious. The pairwise differences of improvement rate of pain after treatment were statistically significant (all P<0.05),and the improvement rate in extension group is the best. The total effective rate of the blank group was55.00%,the flexion excise group was88.89%while the extension excise group was95.56%, there was significant difference (all P<O.05). Aafter treatment, According to MRI,the paraspinal muscles(L4-5level) areas of two groups were larger than those of before treatment,the difference was statistically significant (all P<0.05);the differences in the paraspinal muscles (L4-5level) area of the flexion excise group which compared with the extension excise group were significant (all P<0.05).After treatment,comparing the atrophy degree of paraspinal muscles,we get atrophy degree of the extension excise group is lower than the flexion excise group,the difference was statistically significant(P<0.01).Conclusion:The extension exercise in terms of flatback deformity correction, enhanced lumbar muscles function and mechanical stability to ease pain is more effective than the flexion exercise, there are differences in efficacy.
