Objective: Gout is a group of clinical syndrome due to long-termhyperuricemia, crystals of monosodium urate(MSU) deposition in the tissuesaround joints,epidemiological investigation reveal that the population ofprimary Gouty arthritis(GA) have a upregulated prevalence rate of insulinresistance(IR). Some investigation also cofirmed that, inflammation,autoimmune disorders, heredity susceptibility and metabolic disturbance takepart in the pathological process of the GA. animal model and experimentshave demonsrated that, Elongation of very long chain fatty acids protein6(ELOVL6) may play a role in IR, participate in the progress of Some gene ofinflammasomes and inflammatory factors.So,we want to investigate thediffirent expression levels of ELOVL6mRNA and protein in the peripheralblood mononuclear cells (PBMCs) of patients with GA and the controlpopulation, and clarify the relationship between the gene and proteinexpression levels and the pathological mechanism of the disease.Materials and Methods:(1)274GA(AGA:158,NAGA:116)and112healthy controls (HC) were enrolled into our study. Collected the clinical dataof primary gouty arthritis patients and healthy controls. PBMC were isolatedfrom blood in vitro.274GA grouped by5subgroup according to patients’sfamily history:whether or not suffer from type2diabetes(T2DM),hypertension(HBP),stroke,and according to the patients whether or not sufferfrom uratoma,T2DM,HBP(2) ELOVL6gene and protein expression was measured by Real-time polymerase chain reactin (RT-PCR) and WesternBlot(WB) in PBMCs respectively, the differences of gene and proteinexpression in two groups were compared.(3) Chemiluminescence was used todetect the level of serum insuling.Results:(1)The level of ELOVL6mRNA was reduced in GAgroup(0.00159±0.00061vs0.00262±0.00227,P<0.01), AGA<NAGA (0.00132±0.00069vs0.00205±0.00147, P<0.01); consequently, protein expressionwas also lower in the GA group (0.78±0.31vs1.27±0.44, P<0.01).First-degree relatives of gout patients’ ELOVL6mRNA was higher Comparedto those without a GA family history(0.00404±0.00417vs0.00123±0.00129,P<0.01);First-degree relatives of T2DM,HBP,stroke,their ELOVL6mRNAwas lower than those without a family history(0.00135±0.00127vs0.00226±0.00182,P <0.05); The patients combine with T2DM,their ELOVL6mRNA was lower(0.00129±0.00117vs0.00197±0.00168,P <0.05).(2)Seruminsulin was significantly higher in GA group compared to HCgroup(10.30±3.88vs6.54±2.79,P<0.05).Conclusion: Altered expression of ELOVL6may take part in thepathogenesis of gouty arthritis through immune, inflammatory and metabolicpathological mechanism.
