Objective:To observe the effect of Buxueyimu Granles (BXYMG) on endometrial blood supply and systemic calorific value in patients with the history of spontaneous abortion or in vitro fertilization-embryo transfer (IVF-ET) implantation failure (Qi and blood deficiency with blood-stasis syndrome).Method:Patients with a history of spontaneous abortion or IVF-ET implantation failure (Qi and blood deficiency, and blood-stasis syndrome) in the out-patient department of gynecology, the Second Affiliated Hospital of Chengdu University of Traditional Chinese Medicine, were collected from July,2012to April,2013. They were assigned into group A (oral medication) and group B (oral medication plus enema administration). Each patient in Group A was given BXYMG24g with warm water (provided by Hunan Zhuzhou Pharmaceutical Corporation ltd. Approval NO Z20025791), twice daily. Each patient in Group B was given the same medication as group A, combined with24g BXYMG by enema administration once daily at night. Both treatments were started from the first visit, and the course was one month, in which withdrawal of medication was suggested during menstruation.For patients included from June,2012to December,2012, the Endometrial thickness, peak systolic velocity (PSV) and resistance index (RI) of the distinctive blood flow (nearby the endometrium) at uterine anterior wall and posterior wall were measured by Madison X8color ultrasound and vaginal probe6.5MHZ, during follicular phase (10th to14th days) and luteal phase (20th to23th days). Furthermore, the PSV, RI and PI of endometrial blood flow in spiral arteries in patients from January,2013to April,2013, were measured by Madison ACCUV1XV10color ultrasound, Vaginal probe5.0MHZ.The healthy volunteers and patients were scanned after taking warm water and BXYMG (12g, same volume),30min,70min,100min,130min and160min later respectively by Beijing Beiyi TSI-21thermal tomography imaging system (TTM). Then patients included from June,2012to December,2013were scanned by Beijing Yuetian HIR-2000intellectual infrared thermography instrument before and after medication respectively.10to11whole body thermal images were collected in each scan, and80scans were achieved. Thus the thermal values of lower abdomen, the uterus, Shenque point, Du Mai, bilateral upper arms and upper back were analyzed.Finally, items were observed, including TCM syndrome improvement, thickness of endometrial, blood flow in and nearby the endometrium, and thermal radiation values of lower abdomen, uterus, Shenque point, Du Mai, bilateral upper arms and upper back. Data were processed by SPSS17.0statistical software.Result:There were86samples collected and78samples analyzed in total since8samples of endometrial blood flow during follicular phase being observed were not adequate for statistic process.Among48samples observed endometrial nearby blood flow, there were26in group A (10of follicular phase,10of luteal phase), and22in group B (16of follicular phase,12of luteal phase). There were no significant difference between the age (33.46±4.90,33.09±3.07), menstrual flow, maternal frequency (induced abortion14,8; miscarriage28,18; IVF-ET implantation failure11,6), P>0.05.Among38samples observed endometrial blood flow, there were19in group A (4of follicular phase,15of luteal phase) and19in group B (4of follicular phase,15of luteal phase). There were no significant difference between age (32.0±4.80,32.60±4.44), menstrual flow, maternal frequency (induced abortion15,9; miscarriage12,11; IVF-ET implantation failure2,6),P>0.05.1.The effectiveness of TCM syndromes:87.8%effective rate in group A,91.9%in group B (P>0.05). The improvement on menstrual color and texture, abdominal pain and stabbing pain, and lassitude in group B was predominant than those in group A, P <0.05.2. The thickness of endometrium in group A during ovulation was significantly higher (7.43±2.33,9.44±2.46), P<0.05. 3.The PSV and RI in endometrial nearby blood flow of uterine of anterior and posterior walls during follicular and luteal phase after intervention showed no difference (P>0.05).4. The RI value of endometrial blood flow after medication was significantly decreased in group A (0.67±0.14,0.59±0.09), P<0.05. The PSV, PI and RI values in endometrial blood flow of both groups after medication showed no difference (P>0.05).5. BXYMG increased the calorific values on lower abdomen, left upper arm and upper back at the timing of30min, without affecting uterine calorific value. However, it increased the uterine calorific value in the timing of70min and100min. BXYMG increased the calorific value of the left upper arm and upper back in all5periods.6. The average calorific value of Shenque point after medication increased (31.05±0.89℃,31.66±0.65℃), P<0.05. The minimum calorific value of theleft upper arm after medication increased (21.85±1.99℃,24.47±1.84℃) P<0.05. The calorific value of lower abdomen, uterus, Du Mai and upper back after medication increased, P>0.05.Conclusion:1.BXYMG is capable of increasing endometrial thickness, improving uterine blood flow, and increasing the calorific value of lower abdomen and the uterus.2. BXYMG might improve the blood supply of the brain and myocardium.
