Objective:To investigate the clinical application and manifestation of diffusion weighted image (DWI) and’H-magnetic resonance spectroscopic image(1H-MRS) in differentiating cerebral infarction and demyelinating encephalopathy。Methods:The images and clinical data of53cases with demyelinating encephalopathy and48cases with cerebral infarction were retrospectively analyzed; ADC values were calculated in the ischemic and demyelinating lesions, The rations of Cho/Cr、NAA/Cr were measured in the disease area and contralateral parenchyma respectively, All date was analyzed by SPSS18.0, value was set as P<0.05.Results:1.ADC value of cerebral infarction and demyelinating encephalopathy were5.18±0.80and9.22±1.19, respectively, There was a statistical difference between them (P<0.01)。2.NAA/Cr value of infarction area (1.36±0.11) were lower than in the symmetric health part (1.78±0.09)(P<0.05), NAA/Cr value of demyeliantion area1.57±0.13were lower than in the symmetric health part1.78±0.18(P<0.01), Lac/Cr value (0.32±0.28) compared with the symmetric health part (0.12±0.13)(P<0.01)。3.Cho/Cr value of infarction area were1.39±0.16,compared with the symmetric health part1.59±0.20, There was no significant difference between them (P>0.05); Cho/Cr value of demyeliantion area were1.53±0.44, compared with the symmetric health part1.22±0.11, There was a statistical difference between them (P<0.05)。4.Cho/Cr and NAA/Cr value of cerebral infarction werel.36±0.11and1.39±0.16, respectively, and those of demyelinating encephalopathy area were1.57±0.13and1.53±0.44, respectively, There was a statistical difference between them (P<0.01)。Conclusion:1.ADC values and the rations of Cho/Cr±NAA/Cr in the ischemic and demyelinating lesions were different。 2.DWI and1H-MRS play important roles in the diagnoses of cerebral infarction and demyelinating encephalopathy, To some extent,it also can be used to guid the clinical treatment options。...
