| PartIDepression scale analysis of non-organicgastric motility disorderObjective: Gastric dynamics and its related diseases has become acrucial part of gastric diseases. With the increasing number of patients withdepressive disorder,incidence of functional gastric disorders caused bydepressive disorder is gradually increasing. In this section20patients withfunctional dyspepsia were obtained by endoscopy and other relevantexaminations,further line of barium meal follow through examination,gastric electrical testing and self-rating Depression Scale score,to analysisgastric movement of functional gastric disorders and the combination ofdepression in patients with FD and then a reasonable and timely treatmenthad been given.Methods:Enrolled outpatient and inpatient patients at the Departmentof Gastroenterology of First Affiliated Hospital of Dalian MedicalUniversity from March2012to April2013for the study. By barium mealfollow through examination, gastric electrical testing and self-ratingdepression scale, the level of gastric motility and FD combined depressionof20patients with FD and20normal group were determined.Results: Gastric tone and gastric motility of patients under bariummeal follow through examination in the FD group was weakened comparedwith the normal group, however gastric emptying time of patients in the FDgroup was longer than the normal group(5.59±0.58h vs1.69±0.08h,P<0.05), EGG of patients in the FD group were different compared with that in thenormal group (P<0.05);20patients diagnosed with FD were determined byrating depression scale score and analyzed,and depression prevalence was30%.Conclusion:The gastric motility of patients suffered from FD wasreduced;30%of patients suffered from FD was associated with depression.PartIIThe change of natriuretic peptide signal pathway inDepression-induced gastric motility disorderand the intervention of XIAO YAO SANObjective:This part of the experiment is under the premise beforeclinical trials. To investigate the change of NP signal pathway in depression-induced gastric motility disorder and the intervention of XIAO YAO SAN,immunohistochemistry, RT-qPCR and Western blot were employed.Methods:75male SD rats were provided by the Experimental AnimalCenter of Dalian Medical University. Among these50ones were selected byopen field testing. Divide the rats into five groups:A,B,C,D and E(Followed by normal group,depression rats group,Low,Middle,Highconcentration XIAO YAO SAN group)after1week of adaptive feed. Theexperiment continued for3weeks. Group A was feed normally.GroupB,C,D and E were suffered from classic chronic comprehensive stresstype,that is, given a kind of stress stimulus(water deprivation test for24h,tail clamp,restraint stress) each day. And each was repeated for2~3times.Group C,D and E were perfused intragastrically with low,middle andhigh one hour before stimulus. Group A and B were perfusedintragastrically with normal saline at the same time.After3week, all ratsexecuted, using immunohistochemistry, RT-qPCR, Western-blot technologyto determine the expression of rats C-type natriuretic peptide(CNP) andtheir receptor B-type natriuretic peptide receptor(NPR-B).Results:Depression rats gastric antral circular muscle spontaneous contractions significantly increase the CNP suppression reactionsensitivity,which was improved by XIAO YAO SAN and XIAO YAO SANhas improvement to it.The expression of CNP and NPR-B in depression ratsgroup was significantly higher than that in normal group (P<0.01); Highconcentration XIAO YAO SAN group were no statistically significantcompared with normal group;Middle and high concentration XIAO YAOSAN group were significantly differences compared with depression ratsgroup(P<0.01);Low concentration XIAO YAO SAN group was significantlyhigher than normal group and depression rats group(P<0.01).Conclusion:Up-regulation of the NP signal pathway is one of thereasons that depression cause gastric motility disorders;XIAO YAO SANimprove depression-induced gastric motility disorder by down-regulatingthe NP signal pathway. |
