| Objective: Proximal humeral fracture is common clinical surgicalfractures occur in humeral base more parts of the greater tuberosityfracture, because the elderly osteoporosis easily, because this seesmore at old people comminuted fracture, in this paper, the humerussurgical neck and adjacent to the structure of the correspondinganatomical basis of the research, for the clinical treatment of proximalhumeral fracture to provide the anatomical data.Materials and methods: This article selects40(25cases in menand15women) used for the teaching of antisepsis corpse specimen, theanatomical study on the humerus surgical neck. Observation andmeasurement in spin before brachial arteries and the intersections of outerdiameter to shaft of humerus, the brachial artery before starting placeand with the shaft nodules fixed-point distance intersection spacing anddistance from shoulder seam edge of collar bone end, as well as the spinbefore brachial artery with the long axis of the shaft Angle, therebyfixing the screw before the brachial artery, and the determination of thedistance of the axillary nerve with corresponding osseous marks, thus tospin the location before the brachial artery and axillary nerve research. The results:1. Before the brachial artery in the beginning of2.83±0.44mm in diameter, a maximum of3.26mm, the minimum value of2.50mm;Spin before brachial artery with the long axis of the shaft Angle is73plusor minus five degrees, the maximum is78degrees, the minimum is68degrees.2. Before the spin brachial artery and brachial backbone nodedesignated distance from nodules as: man:25.38±0.52mm,25.88mmmaximum, minimum value is24.88mm; Female:21.77±0.64mm, amaximum of22.40mm, the minimum value of21.14mm; Distancefrom the shoulder seam edge of collar bone end is: man:54.36±1.52mm, a maximum of55.86mm, the minimum value of52.86mm;Female:46.78±1.44mm, a maximum of48.20mm, the minimumvalue of45.36mm.3. The length of the former branch of axillary nerve trunk was31.45±0.98mm,32.42mm maximum and minimum value of30.50mm;Maximum width of2.21±0.03mm,2.24mm, the minimum value of2.20mm; Maximum thickness of0.43±0.04mm,0.46mm, theminimum value of0.40mm.4. Axillary nerve in front of the shoulder peak Angle of the deltoidtuberosity in attachment and the distance away from the apex of thegreater tuberosity is: man:45.27±1.38mm,46.64mm maximum andminimum value of43.90mm; Female:39.56±1.32mm, a maximum of40.88mm,the minimum value of38.26mm; Distance from the shoulder seam edge of collar bone end is: man:60.15±2.57mm, a maximum of62.70mm, the minimum value of57.58mm; Female:54.76±2.38mm,a maximum of57.12mm, the minimum value of52.40mm.5. Prior to the branch of axillary nerve into the muscle point from thedeltoid muscle leading edge distance of31.37±0.98mm,32.34mmmaximum and minimum value of30.42mm; Distance from the acromionedge of collar bone end of49.54±1.33mm, a maximum of50.86mm,the minimum value of48.22mm.6. Prior to the branch of axillary nerve in front of the shoulder peakAngle of the deltoid tuberosity and the attachment of1.82±0.07mm inwidth, the maximum of1.88mm, the minimum value of1.76mm;Maximum thickness of0.32±0.03mm,0.34mm, the minimum valueof0.30mm.Conclusion:1. In the deltoid muscle pectoralis major surgicalapproach, the gully between the acromion edge of collar bone end about4cm carefully free organization avoid damage before the brachial artery.2. Before the spin brachial artery on the shaft of oblique contorts, when humerus surgical neck fracture, the fracture line should payattention to and spin before brachial artery contorts the relationship.3. In the deltoid muscle lateral approach, free muscle tissue in theprocess, about5cm at the top of the free from nodules should be payattention to protect the former branch of axillary nerve. |
