| Research Objective:The famous professor of traditional Chinese medicine Zhou Zhongying proposed "damp,heat,stasis and toxic" is a basic pathogenesis of chronic kidney diseases (CKD) on the basis of his60years of clinical practice. According to this theory, a study of exploring Stagnant Heat pathogenesis evolution law and other relational factors can be established through clinical epidemiology survey and establishing clinical epidemiology database of Stagnant Heat pathogenesis of chronic kidney diseases. As a result, the dialectical understanding and treatment of traditional Chinese medicine for CKD can be improved.Research Methods:From July,2012to December,2012,200cases in accordance with the diagnostic criteria of chronic kidney diseases are collected into the study,which are from Jiangsu Province Hospital of TCM. The TCM syndrome characteristics of chronic kidney disease, the law of Stagnant Heat pathogenesis evolution and the relationship with each factor all can be explored by way of using SPSS17.0software of statistical analysis.Research Results:200cases of CKD are collected in this study including53cases of stagnant heat pathogenesis which consist of31cases male and22cases female. By calculation we get the result of P>0.1. There is a significant difference between different ages in patients of stagnant heat syndrome. The highest rate is38.462%in the patients from50to59years, on the contrary, the lowest rate is8.889%in the patients older than70years. By calculation χ2=12.318,so the result of P is less than0.05.When comparing different periods of CKD with the frequency of stagnant heat syndrome, the percent of5period of CKD is higher than others by calculation of P>0.05.Different personal habits such as tobacco, alcohol, high-salt foods, high-fat food have statistically significant differences in the incidence of stagnant heat syndrome of chronic kidney diseases by calculation of χ2=7.95711and P<0.025.The frencency sequentially is high-salt foods100%, fat50%, smoke34.615%and wine33.338%. In the wake of different diseases accompanied with stagnant heat syndrome of chronic kidney diseases,the incidence of stagnant heat syndrome is also different. The common accompanied diseases can be SLE(71.429%),Gout(57.143%), coronary heart disease(37.500%), hypertension(29.545%), diabetes(26.425%) and cerebral infarction(15.385%).The pathogenesis of stagnant heat syndrome of chronic kidney diseases often combine with dampness(81.132%), deficiency of Qi(67.925%),deficiency of Yin(35.849%),deficiency of Yang (7.547%)and phlegm(1.887%).Sweating, lumbar pain, mental irritability, urine bubble, flushing, dysphoria, polyuria, dizziness, claw violet dark, decreased urine output, anorexia, lower extremity edema and body heat are the common manifestations which turn a high frequency (>40%) in patients with stagnant heat syndrome of chronic kidney diseases.Research Conclusion:(1) The basic pathogenesis of chronic kidney disease is deficiency in origin and excess in superficiality. Qi deficiency syndrome is the most common of the internal syndromes. At the same time, stagnant heat syndrome and dampness syndrome are more common of the external syndromes.(2)There is a significant difference between the cases of stagnant heat pathogenesis of CKD with different ages, different eating habits and different associated diseases.(3) The patients of stagnant heat syndrome are often accompanied with dampness syndrome.(4) The red pale, irritability and dark purple nails are regarded as the main clinical manifestations of stagnant heat syndrome of chronic kidney diseases. As a result of this study, the manifestations of Qi deficiency syndrome such as dizziness, shortness of breath, fatigue and weakness can also be found at the same time. |
