| Objective: To evaluate the modified four inverse symptomatic efficacy of Chinesemedicine, Decoction on chronic congestive heart failure patients with heart deficiencysyndrome integral, Lee’s failure classification of cardiac function, serum NT-proBNP,echocardiography and other indicators of the intervention effect and mechanism of action.And the safety of the drug were evaluated before and after treatment.Methods: patients from2012March to2013March in Shandong Provincial TraditionalChinese Medical Hospital emergency, inpatient and outpatient ICU. Participants werediagnosed with chronic congestive heart failure heart and kidney yang deficiencysyndrome patients, randomly divided into groups, the treatment group in30cases, in30patients of the control group. Two groups were given conventional treatment of Westernmedicine based on. Treatment of two groups of patients are followed in2007"chroniccongestive heart failure diagnosis and treatment guideline", conventional selectiondiuretics, ACEI, beta-blocker therapy, when necessary, using digitalis therapy. The controlgroup was given conventional treatment of Western medicine: active treatment of theoriginal diseases, improve the ventricular remodeling, decrease the heart load; test groupwas treated with Jiawei Sini Decoction in four based on the above treatment, treatment for4weeks. The observation of two groups were compared before and after treatment of TCMsyndrome integral, integral western Lee’s failure, heart function, echocardiography(including ejection fraction, stroke volume), changes in liver and kidney function beforeand after treatment the serum NT-proBNP and treatment hepatorenal function change.Results:1.the TCM syndrome total score two groups after treatment, symptoms oftraditional Chinese medicine treatment group, the total efficiency is86.70%, control group,the total efficiency of60%, the total effective rate in treatment group was significantlyhigher than that of the control group, p<0.01, statistically significant difference, thetreatment group in improving patient symptoms significantly better than the control group.2.Comparison of cardiac function in of two groups after treatment, the totaleffective rate in treatment group was93.30%, and no heart function deterioration cases. The control group the total effective rate was80%, the total effective rate in treatmentgroup was significantly higher than that of the control group, p<0.05, statisticallysignificant difference, the treatment group in improving cardiac function than the controlgroup.3.Comparison of patients with NT-proBNP, two patients after treatment,compared with before treatment, p<0.01, there was significant difference; after treatment,the treatment group and the control group, p>0.05, has no difference before and aftertreatment; difference comparison, p<0.05, there was significant difference between thetreatment group, NT-proBNP levels decreased significantly, suggesting modified four Sinidecoction could obviously reduce the NT-proBNP level in plasma of patients with heartfailure.4. Comparison of single symptom score of after treatment in the two groups,the treatment group in the shortness of breath, palpitation, fatigue, limb edema, oliguriasymptoms than before treatment were significantly improved, p<0.05or p<0.01; thecontrol group in the shortness of breath, swollen limbs, oliguria and before treatmentimproved, p<0.05; the two groups, the treatment group in improving the shortness ofbreath, palpitation, fatigue, swollen limbs symptoms than the control group, p<0.05.5.Comparison of integral Chinese Lee’s failure in of two groups aftertreatment, the total effective rate of treatment group was86.70%, control group, the totalefficiency of60%, the total effective rate in treatment group was significantly higher thanthat of the control group, p<0.01, statistically significant difference, the treatment group inimproving symptoms significantly better than the control group.6.the treatment group of three conventional, liver and kidney function andother safety indicators before and after treatment, no significant difference, p>0.05.Conclusion: modified four Decoction in reducing the integral, TCM syndrome inpatients with heart failure in general integral, cardiac functional grading and reduce plasmaNT-proBNP levels in patients with heart failure compared with the control group, there wasstatistical significance, that this can relieve clinical symptoms, improve exercise toleranceof patients, improve the quality of life in patients.Deficiency of heart qi is the initial factor of heart failure, yang-qi deficiency is the key pathogenesis of congestive heart failure,heart and kidney yang deficiency is an important pathogenesis of heart failure andaggravating factors, Yiqi Wenyang Huayu Lishui, is an important measure in the treatmentof chronic congestive heart failure.Keywords modified four Sini... |
