| Objective: By comparing open reduction and internal fixation with manipulativereduction and splint therapy in elderly patients with intra-articular distal radius fractureswrist function and complications, explore the treatment of choice in elderly articular distalradial fractures.Method: In this study, cases from the Affiliated Hospital of Shandong University ofTraditional Chinese Medicine orthopedic ward, emergency and outpatient services.Admitted from October2010to October2012,70cases of patients with type C fractures ofdistal radius. Closed reduction and small splint treatment of31cases (non-surgical group),open reduction and plate fixation for the treatment of39cases (surgery group).After6months and12months, respectively, of all types of according to Cooney wrist functionscore, each group of patients with wrist function score compared with the statisticalanalysis and comparison of the two treatment methods complication rate.Result: The surgery group incisions â… healing. The patients were followed up for3-24months, an average of14months. X-ray showed two sets of fractures healed, Fracturehealing time: the non-surgery group (10.8±2.0) weeks, the surgery group (11.7±2.5)weeks. Last follow-up surgery group volar tilt, ulnar deviation, and radial length ofrecovery is better than the non-surgery group, Cooney wrist rated non-surgical group wereexcellent in9cases, good in16cases,4cases and poor in2cases, good rate of80.6%;surgery group were excellent in10cases, good in19cases,7cases, poor in3cases,good rate of74.4%. Articular surface â… , grade II injury was no significant differenceexcellent the III level damage excellent rate of non-surgical group than the surgery group,and the incidence of complications than the surgical group. Conclusion: For elderly type C fractures of distal radius articular surface of grade â… ,grade II injury patients, Manipulation splints after treatment, the clinical efficacy with nosignificant differences in the clinical efficacy of open reduction and internal fixation of;For articular surface grade â…¢ injury patients, long-term clinical efficacy of conservativetreatment is superior to surgical treatment, surgery and the complication rate is lower thanthe conservative treatment group. Although the degree of anatomic reduction after closedreduction splints treatment such as open reduction and internal fixation, but can still get thewrist for elderly patients with articular distal radial fractures. |
