Objective:To investigate the effect of internal and external treatment with the Chinese Medicine, particularly prescription of Tongluotangtai and washing application of Tangtongwai solution, on Qi and Yin deficiency and blood stasis block syndromes of DPN patients, including clinical symptoms, nerve conduction velocity, IGF-1, TNF-a, blood glucose, glycated serum protein and other indexes; evaluate the effect of external treatment with the Chinese Medicine on DPN patients; provide a theoretical basis of the clinical application.Method:62patients with DPN were randomly divided into the treatment group with Chinese medicine and the control group with Western medicine. All patients were treated with the basic therapy of diabetes mellitus. The patients in treatment group were given Tongluotangtai (150m tid,3times a day,8weeks) and Tangtongwai solution washing the legs (15-20minutes, every other day) while the patients in control group were given Mecobalamine (0.5mg tid). The symptoms of patients in two groups were observed before and after the treatment, including clinical symptoms, plasma glucose, glycated serum protein, sensory and motor nerve conduction velocity, IGF-1, TNF-a and other indexes. Results:The cliniical curative effective of symptoms in the treatment group:the significant effective rate was50%, the total effective rate was96%, while in the control group27%and77%. There was a remarkable discrepance (P<0.05) between the two groups. Clinical syndromes and signs were significantly improved in the treatment group, including sensory and motor nerve conduction velocity were significantly improved, IGF-1increased, TNF-a decreased, plasma glucose and glycated serum protein improved. Conclusion:Internal and external treatment with the Chinese Medicine can obviously improve the clinical symtoms of DPN patients, and improve sensory and motor nerve conduction velocity, increase IGF-1and decrease TNF-a. This treatment has curative effect for diabetic peripheral neuropathy.
